Brief Answer:
Control of stress and / or anxiety at any cost

Detailed Answer:
Hi Madam. Thanks for your query and an elucidate history which I read and re-read not to miss any of the points you have mentioned in the Document. (by the way these are 2 pages and not three- might be computer has adjusted it to 2 pages, please check).

I can understand the plight one goes through with the advancing age, with the happenings in life, sometimes so fast as to coping becoming difficult; the disturbed psyche not getting enough time to recover form the previous one and you get the another. You have mentioned everything so chronologically and categorically.

Noted your problems of bloating, burping, PPI and H2 blocker, burping on pressure, pains at many site in the body for 3 years; only 20 days were the asymptomatic for your. The sleep and food pattern and everything you have mentioned.

My thoughts:
The commonest causes of gases, bloating and burping are as follows and something or the other will be co-related to your condition :
-Aerophagia- inadvertent swallowing of the air is the commonest cause and occurs so unknowingly that it is difficult to be accepted by the patient.
-Intolerance to certain foods and beverages including candies
-Psychological upheavals
-Small intestinal problems (your motility studies were normal)
-Hiatus hernia (in the past but settled with weight reduction).
-Loose dentures, and so on.

As I could think of in your case, the Doctors whom you consulted must have thought about these causes but might not have been open to say so to you for whatsoever reasons they only know of.

I would suggest you the Following:
First of all you have to have a control of your stress and /or anxiety level. This can be done with the ways you know the best- includes Meditation, hobbies, outings, keep away from all the factors that increase your stress and anxiety. Or can be helped with medicines for which you will need to consult a Doctor who should have been conversant with and would add mild medicines to have reasonable control over anxiety and / stress.
I am hopeful that this alone should solve your problem to the largest extent.

I know it is very difficult to accept many things at this point of time and age. But one has to.

Symptomatic medicines in the form of:
Motility regulator like Domperidone or Metoclopramide strictly in consultation of your treating Doctor.
H2 Blockers really work well , but will help only to control the acidity , not the burping.
Please do not try different tea and such things as they can cause more harm in some cases , as you also have mentioned.
Activated charcoal tablets, probitics sometimes may help
A good sleep can help reduce your problems.

I hope this answer helps you in decision making and sure success in controlling your symptoms and have a better life.
Please feel free to ask for further relevant queries if you need to or if you feel that there is a gap-of-communication.

I will be very happy to assist you till you get symptom-free.
