Suggest Treatment For Severe Depression

Question: Hello Doctor(s),
My son is in 2nd year of his B.Tech. He lives in a hostel attached to his University. I am very much concerned about his behavior and attitude. I fear he might be facing depression. Some of the behavior we find out of way are:
1) He is not able to mix with other students
2) He has almost no friends. He tells that his friends very often tease him and make mock of him, so he tries to remain alone.
3) He is having negative feeling about life, friends.
4) His confidence level is low
5) He is introvert type so doesn't comes forward and asks for clarification of doubts with his teachers.
6) He things that all the bad things happens with him only.
7) He mostly prefers to play games in his XXXXXXX instead of talking with family members when he is at home during vacations.
8) When we try to convince him about anything, he generally feels offended.
9) He is not performing well in his studies.
10) Recently when we the whole family went for a outing, he was not enjoying. He felt frustrated even we click snap during the visit.
I recall that he had told us that during the school days, there were 2-3 students who used to tease him and physically hurt him, In my opinion that may also be the root cause from where his behavior became negative,
Please advise what best can be done, as I fear that this may lead to a depressing situation. Hoping for your kind attention and proper guidance / advise. He is studying in Jaipur(Raj)(India) and I am living in Kota(Raj)(India). Thanks and Regards.

You have very well narrated his symptoms.

From history it seems that he is likely suffering from depression.

Other possibilities are social anxiety disorder, personality disorder or some kind of psychotic problem.

In social anxiety disorder person always thinks that in social situation he/she will be criticized or observed by others for his acts. This leads to reduction in performance and avoiding the situations.

In personality disorder onset occurs in ado scent age and gradually symptoms increased and after onset of adulthood they become prominent affecting social or occupational life/study. Possibility of avoidant personality disorder or anxious personality disorder is there.

In psychotic disorder person is scared of people. He/she feels that someone is harassing or doing something bad to him though in reality nothing is there. Person becomes suspicious and avoids the social contacts due to fear.

Yes you are right that incidents happened at school may be associated psychologically with current problem.

For exact diagnosis detail history and mental status examination are needed and these should be done by a psychiatrist. After diagnosis treatment can be done in form of medicines and psychotherapy.

Consulting psychiatrist soon is the best option at present.

Thanks for your response Dr XXXXXXX Solanki. It seems the best solution is to an advise of a psychiatrist. I will try to meet a Psychiatrist, but this will be possible when My son comes home during a vac cation or when we go to meet him. Till then please advise me what care should be taken while taking to him, or what type of talks will benefit him, so that he gets motivated and doesn't goes to deep depression.
Also if possible please suggest names of some good Psychiatrist in XXXXXXX and XXXXXXX who can help us out of the situation in the quickest mode. Thanks and Regards.

Dear XXXXXXX

Thanks for follow up and sorry for delayed response. Considering his situation I recommend consulting doctor as soon as possible. This will prevent worsening of his problem as well as academic performance. Earlier the treatment better and faster the response will be there. Delay in treatment may affect his study which then can act as stress.

As such it is not possible that merely talking you can treat or prevent deep depression. Still I want to mention some points.
For a while ask him to maintain his schedule of meals, sleep and study. He should do exercise if possible for at least 15 minutes. Ask him that if he faces any problem, he should report to you without any hesitation as you are always there to help him.
Encourage him to take part in sports activity or another group activity in which he gets social exposure.
If you are in contact with any of the friends who can give details about his behavior in hostel and college, then you can take help from him. You should meet that fellow and explain about his problem and ask him to help/take care him if possible in polite manner. Physical presence of support matters much. In any normal or psychologically disturbed person, sharing the thoughts and feelings gives relief.

Thanks Dr XXXXXXX Solanki for the response. We will meet a Psychiatrist as soon as possible. I need just a one more advise. Meeting a Psychiatrist indicates that there is some psychological problem. I fear that the feeling of having some psychological problem will may further add to the depression of my son, how to deal with that? On the first hand he will not feel comfortable and he will not be ready to meet a Psychiatrist. Thanks and Regards,

Thanks for asking your doubt.

Yes I can understand your apprehension. In our society stigma against psychiatric illnesses are still prevalent but on the other hand there is also much change in attitude of the people for seeking psychiatrist consultation for problem.

First of all you should be clear that there is need of consultation as his problem is psychological likely. You yourself mention that he should not undergo deep depression means there is something which is related to mind. To treat any problem person must accept that he has problem. Being a parent you should not have fear that feeling of having some psychological problem will may further add to the depression. It is not like that. Rather not taking treatment at appropriate time may worsen the problem.

All persons taking treatment for psychiatric problems are not insane. Most of the psychological problems are treatable with current medications and psychotherapies.

He is educated and doing his engineering. In my opinion he can be convinced for consultation at least. Just explain him that, '' Dear Son, I feel that you are in some stress and that is affecting your study and life and we are observing that. We feel that you should come out of this and we should consult doctor for counselling at least.'' After consulting once I think it is duty of psychiatrist to make him ready for treatment and carry out further treatment.

Psychiatric problems have also some brain pathology in form of neurotransmitters disturbances and person who has brain may suffer from psychiatric problem. So it is better to ask for help before it worsens.
