Suggest Treatment For Severe Constipation

Question: Hello Doctor,
So what shall I do? My doctor has told that I do not need any medicines,also I have never had dry or hard stools as in the case of constipation.I always believed the stools are not moving properly,but when I used to get a bowel movement,the stools were always normal. After the cleanse I am having daily bowel movement.But I have some force or pressure,it feels like some air pressure is just moving around my intestines,what shall I do for this,it is hard to bear that.It is not that I have a bowel movement,and I feel perfectly alright.I want to what is that and how can I overcome this. Thanks

Going by your case history, that stools are normal only there is feeling of something in the intestines so it seems likely to be “GULMA” [phantom tumor/abdominal lump], and the most important causative agent in the pathogenesis is ‘Vata Dosha’ so line of treatment & management is as follow:

1. Panchkarma
. Snehana [oleation]: with oil as described earlier
. Swedana [sudation]: consult your attending Physician
. Virechana [mild purgation] with olive oil daily
. Basti: decoction enema

2. Medicine which you can take:

i. Hingvashtaka Churna: 3 gm mixed with little ghee [with first bite of meal]
ii. Dantadyarishta or Pippaliadi Assav/Kaumarya Assav : 20 ml after meals twice daily
iii. Hingutriguna taila: 6 gm with milk/warm water
iv. XXXXXXX vati: 2
Gandhak vati: 2
Chitrakadi vati: 2……………. With warm water thrice daily.

3.
i. Take: old brown rice,horse gram,ash gourd,amla,pomegranate, ginger,castor oil,cumin seeds,asafoetida, easily digestible & carminative

ii. Avoid: heavy/slimy food, incompatible food, dry fruits/vegetables, tubers and root,fish, and suppression of natural urges.

PS. could be simple gulma [gaseous tumor

Hello Doctor,

Can you list few symptoms of Gulma so I can further consult my doctor regarding this. As in tumor you are talking about,so it needs to be present at one location ? I feel something is trapped in my abdomen,and it is causing me to not evacuate my bowels completely,until they are very soft or kind of liquid types.So if it is of solid and kind of dry,I become bloated and am not able to pass,so I have stopped eating foods that produce more gas,which list the doctor has already given me. I have been suffering from this since last 4 months.Before that too,I had some digestive issues,but after taking triphala or something else,I used to feel better.Now even after having a bowel movement,I do not feel relieved.I have visited many doctors since many months.But I think no has been able to diagnose my main problem.I have contacted 3 ayurvedic doctors since april,2 Gastroenterologist.But I dont think I have been properly diagnosed.I think till the time I am not properly diagonsed,how can I find what is wrong with me,and accordingly follow my lifesetyle. Last 4 months have been worse for me. I am not able to concentrate on my work,as it is very disturbing.My sleep is disturbed because of this.I do not feel refreshed.Few said,it could be anxiety.But am sure it is not at all anxiety or anything of that sort.I think my line of treatment has not been proper,so how can the cure be proper. What are the ways to find out exactly what is wrong with me and then move on further. I eat very light food,warm,easily digestible foods,nothing raw,but I cannot see much difference.Thanks for the help.

1. Since ‘gulma’ is very difficult to diagnose thus the word tumor is used in general term without referring to the physical size of the mass/nodule/lesion.

2. As tumor can be benign, pre malignant or malignant thus is not similar with cancer which usually means malignant, so it is very difficult to co-relate gulma with a single definition in modern context.
PS. gulma can vary from simple gastritis to more complex abdominal lump

3. As you have visited gastroenterologist twice, were any investigations ordered like:
. Endoscopy
. Blood tests (to look for chemicals such as tumor markers)
. Chest x-ray
. Complete blood count (CBC)
. Ct/MRI scan [if needed]

4.     Since about gulma it is told that it does not give clue about its site of origin thus it will require a meticulous physical examination with face to face interaction with an experienced Ayurveda Physician [probably there is samana vayu vikriti i.e. disturbance in digestion and metabolism]

5.     One might have to think on the line of Avrit Vata Chikitsa [especially samana avrit apan chikitsa]

Hello Doctor,

I had got done my breath test for H pylori infection,but it was normal. No infection.I have uploaded my blood test reports.I had got done my X ray and had told me that faecal matter is stuck in my large intestine.I am not sure if it was faecal impaction.Can you tell me symptoms of faecal impaction ? I think it was not faecal impaction,as I have normal bowel movement when I have it.Yes the bowels are soft mostly lacking in shape. My doctor has suggested me to have a colonoscopy,but said it was mostly IBS,so you wont find anything in colonoscopy,for your sake,if you want to have a colonoscopy you can have it.So i didnt have any colonoscopy. Main thing is I do not know what is wrong with me. According to you what should be my next steps.Shall I do a colonoscopy to check if any tumors or not.As colonoscopy can easily determine about it. I always have some discomfort in my abdomen,I do not know what is that. It is not severe pain,which I just cannot bear it. It is something I cannot ignore it.But also it is not something that I can live with forever.I want to be back to my normal feeling,without any medicines.I will take care of my foods after my doctor suggests what foods to follow.But firstly I should see a considerable improvement for me to believe what was wrong with me.But so far it has not happend.What questions shall I ask the ayurvedic doctor on my next visit to have a thorough diagnosis.Also ayurvedic doctor here didnt check my hand pulse too.I have heard,normally they diagnose the pulse.Thanks.

1. Symptoms of fecal impaction is/are: Liquid stool, weight loss,reduced appetite, nausea/vomiting/headache, abdominal pain [after meals], constant feeling of passing stool [possibly it could be vataj gulma]

2. Yes one can go for colonoscopy since it can be both diagnostic [one can identify the cause of symptoms] or can be interventional [ for screening purpose]
PS. do let me know about the report.

3. You need not to ask about the questions , just tell about the provisional diagnosis made online they will themselves prepare the questionnaire for you.

4. As there are different branches in Allopathy [cardiology,surgery,diabetology, gynecology, paediatrics etc.] in the same way pulse therapy is different, may be the doctor was not specialized in this field.

5. I have gone through your reports and all are within normal limits.
