Brief Answer:
Heart related problem, urgent EKG and evaluation

Detailed Answer:
Hi.
Thanks for your query.
Noted your history of current medications and the present complaints, viz. chest pain below the left breast, light headedness and weakness on standing up. Breathing is heavy. Also noted the history of large para-esophageal hiatal hernia and have cardiomyopathy and seen the medications you are already taking.
Wants to know whether this is the heart or hernia causing the typical pain you have.

I would think of a cardiac reason as the first as per the history you have provided. And go for urgent EKG as the first diagnosis, get the blood tested and get an X-ray chest done to rule out any lung related problems and can also show the intestines in the hernia that have gone into the chest.

This is very important to rule out the cardiac problems first.

I hope this answers your query, please feel free to ask for further relevant query if you feel that there is a gap of communication.
