Hello and thank you for asking HCM.

I have read the symptoms of your child and my answer is as follows:

The symptoms are----

-low grade fever
-pain in the right quadrants of the abdomen
-loss of apetite
-lethargic

It would be great if you can gave me some other info. Did the child vomit ? How are the stools, is it soft/loose motion ?

The immune system of a 4 years old is not mature enough to show all the symptoms of an infection in her abdomen, that's why he/she is lethargic probably.

I'd suggest at this point to a prompt clinical check up with the pediatrician to do a physical examination of the child and do an ultrasound of the abdomen and blood tests to check the white blood cell count.

Appendicitis, gastroenteritis for example have these symptoms and your child needs to be examined by his/her doctor. Based on the physical examination it'll be apparent if you need to take the child to the ER or not.
