Suggest Treatment For Recurring Pus Cells In Urine

Question: Hi there, my father is 68 and has a history of recurring pus cells in urine but recently it showed full field per hpf. He is born with one kidney and has a diabetic condition. RBCs are 8-10/ hpf and specific gravity QNS. His ultra sound of kidney was done and looked normal. Is that critical? What specialist would be good to look at?

UTIs in the male is always considered complicated moreso in the elderly population. How is his urination? Does he have prostate enlargement. BPH or benign prostate enlargement may cause weakness of urine flow and will cause residual urine to remain in the bladder. This causes stagnation of urine in the bladder which will be the cause of bacterial growth.

Another condition is if his kidney that did not develop actually developed but did not mature. This sometimes cause UTI because the tissue of the underdeveloped kidney is prone to infection. A CT scan is needed to check if a kidney remnant still exist and often times needs to be removed if proven to cause the UTI.

You need to see a urologist, we specialize in the kidneys and prostate organ. It is not critical but you should seek an appointment with your local urologist.

Hi there,
Just to bring to your attention that my father visited a specialist yesterday and he was suggested some tests and medicines. He was told that his high blood sugar might be a cause of pus cells. I would appreciate if you take a look at tests and medicines prescribed. Are those fine? What is your reading based on this evidence? Please find new reports as attached.

I saw the reports and it shows that your father has uncontrolled diabetes. I would recommend to see an endocrinologist for proper diabetes control. His urinalysis is loaded with pus cells which may indicate a UTI. I didn't see the result of the urine culture which would help us see what is the organism and what antibiotics is needed.

ultrasound looks good and both kidneys are normal. Uncontrolled diabetes might lead to UTI so proper blood sugar control is essential.

Hi there,

Thanks for getting back to me. I just wanted to make sure with you that my father needs to see endocrinologist only or he needs to see both urologist and endocrinologist for getting his pus cells cured. The medicines prescribed as its pictures uploaded in the file seems to be the one to control blood sugar. Would you like to prescribe any medicine to control his blood sugar level at this point of time? Please note that he is born with one kidney only.

The endocrinologist would be the one to control his sugar by placing him on a diet and adjustment of his medicines. I think he may delay the check up with a urologist since his ultrasound came out normal. I would want to see the result of his urine culture so I can suggest an antibiotic.

In the recent ultrasound it said both kidneys are normal.

I have read the attachments and reviewed them one at a time. This was the same attachments from the previous days.
Medicines that were given to you was for diabetes which needs better control. That is the reason you need an endocrinologist rather than a nephrologist to manage your diabetes. Kidney function are all normal which is very good and it will remain the same if you have better control of the diabetes.

I am happy with the ultrasound result and it is unremarkable. You need to get the urine culture so an antibiotic can be started to treat the infection.
