Suggest Treatment For Recurring Anal Fissure

Question: Hi Doctor , I am a 23 year old guy with the history of recurrence of anal fissure , the probelm has been going on and off for the past SIX years.I got diagnosed when I was 16 from being constipated . bleeding and pain has been present for at least 3 weeks now , other times it would stop withing couple of days and I would be okay for 2 - 3 months and then slight constipation would bring it back . I have seen a Doctor two weeks ago and he said I had internal hemorrhoids and acute anal fissure. He put on Rectogesic and I only started using this cream two days ago , it gives me severe headaches . I have tried so many different creams and they have only worked temporarily. I eat diet rich in fiber , take stool softeners and using currently using Rectogesic . I have included PHOTOS of this terrible condition and PLEASE take a look at them and tell me if anything looks suspicious ? I am worried about Anal Cancer and when I Google anal cancer pictures that come up look like my pictures , I am extremely worried at this point , could the doctor miss something that important during my visit ? I do not understand what is the skin growth outside of my anus ? PLEASE ANSWER AS SOON AS YOU CAN . THANK YOU !

Hi and thank you for asking.
After review all your photos and your concerns I can say as follows:



1. You have hemorrhoids with anal fissure. There is nothing else on photos, there are no suspicions for anal cancer. The skin growth outside of your anus is related with skin and anal sphincter damages during constipation.



2. You are reporting to have headaches after Rectogesic use. It is the more common side effect of it. This is why I recommend stopping Rectogesic and start to use preparation H .



Meantime you can do home remedies to relieve your symptoms like:

- use olive oil three teaspoons per day

- take plenty of water.

- Soak your anal area in warm water 10 to 15 minutes two to three times a day.

- use toilet not less than 30 min

- avoid anal sex

- increase physical activity

- keep dry and clean anal area.

3. If the symptoms still persist after a couple of weeks than you should consult with anal surgeon the possibility of surgery procedures for chronic annal fissure and hemorrhoids.

Thank you For the answer , I have been waiting all day as I was so scared of anal cancer and more serious diseases of that area , The doctor that I got to see two weeks ago also told me the same , acute anal fissure and internal hemorrhoids . so these pictures do not indicate something more than anal fissure and hems ? I wanted to go for the biopsy of the skin but if you think it is not needed I will avoid that procedure ?

With regards to pictures, it is a typical view of hemorrhoids and anal fissure, nothing more serious like cancer or polyps.

To my opinion you don't need to do skin biopsy for two reasons :
- the first that there is no suspicious findings
- the second is that it is an inflamed area and with a lot of blood vessels so the biopsy will show only inflammation cells.
For the moment you need proper treatment and nothing else.
