Question: Hi Doctor,My mum is 58 this year and since last one year she has started severe thoughts of fear of someone trying to kill her including our neighbours. She even thinks that people follow her where she goes even to the extent that when she travelled from XXXXXXX to overseas to stay with me. Even when we go out she keeps on saying this is the person that we saw there and they are following me. I got married this year and she has also started saying my wife is mixing something on her food and she is talking to our neighbour in our mother tongue where bya ll our neighbours are non XXXXXXX Language speakers. Where ever my wife goes she will follow and when she goes to toilet she says she hears that my wife is talking to somebody. I seriously believe this needs medical attention as I tried explaining to her earlier and I couldnt convince her to understand the situation. I have gone through your detail history and I guess it may features of psychotic illness.

Most of the details suggest diagnoses of schizophrenis.

2nd diagnosis is dementia which occur in late age and most of the time associated with this kind of psychotic features.

She has very much suspicious can be diagnosed as delusional ideas. Which is prime feature of schizophrenia and associate feature of dementia.

Schizophrenia has problem in cognition, perception and thought processes.

Dementia has poor outcome and it can not reversible.

Need immidiate psychiatrical attention.

My personal advise to you take her for psychiatrical evaluation.

This condition can be treated with antipsycohtic medicines and prognosis is good when we will do early intervention .

Antipsycohtic medicines like risperidone, olanzapine etc has very good results.

There are various kind of treatment available for psychotic illness and depend upon patient's clinical profile.

So consult psychiatrist and get treatment as early as possible for better and fast recovery.
