Suggest Treatment For Pneumonia, High Fever And Diarrhea

Question: 4yr old son is chronically sick for the last 7 months. pneumonia twice with one hospitalization. Those were 4 months apart. Then only a month after that he had a high fever, vomiting, diarrehea then stool turn bright green then white, low energy and no appetite for about 10 days. He's a very active kid. Now only about 3 weeks later he is running a fever of 103 and has a cough and runny nose. Could there be some underlying reason he's sick so much? Maybe a immune disorder? Should I be concerned or is he just that unlucky?

Questions:
1. What was the birth weight and what is the current weight
2. How is the attaining of developmental milestones
3. Is the stool oily and floats in the pan
4. Is there any color change in the hair or skin
5. Is there any history of recurrent serious infections other than Pneumonia
6. Were the two pneumonias demarcated on chest X-ray
7. Was there clearacne on 1 st X-ray before the 2nd pneumonia occurred.
8. Was it occuring at same place in lungs

Based on the current information the possibilities are:
1. Immunodeficiency either primary or acquired.
2. Congenital lung anomaly.

Two pneumonias occurring 4 months apart definitely needs evaluation immunodeficiency.

Follow up: Dr. Sumanth Amperayani (16 hours later)
no other serious infections. the 1st xrays did show pneumonia in left lung. The 2nd round show it in the lower left lung for sure. his birth weight was 6lbs 12oz and is 36lbs currently. he was hospitalized at 5 weeks for RSV. I hear that they are more prone to getting an infection in the lungs/respiratory after getting RSV. We did go see his doctor this morning and he diagnosed him with croop. Would you agree at this point?

His birth weight and current weight are excellent.

It is true that RSV will have a preponderance to secondarily developing wheezing but not to infections, so do not worry. That too after this long period, it is unlikely. Usually secondary infections are more common during the RSV infection.

Croup is a clinical diagnosis and I completely agree with your doctor as nobody can mistake the classical inspiratory noise during the croup. Moreover one has to listen or the see the video of a child to diagnose or agree or disagree with croup. I suggest to keep your son as comfortable as possible and do not make him cry or agitate as these will aggravate the noise and croup.
