Suggest Treatment For Pimples On Face

Question:
Hi Doctor,

My wife is facing a severe problems of pimples on the face. she had delivered a baby boy 19 months ago and used to feed the baby. She stopped the feeding almost 3 months back now. We have consulted the Skin Specialist and as per skin specialist advice , used the below medication.

1. Azithromycin tablet 500
2. CuteE 400
3. CODOX-L
4. Bellisa Soap
5. NEVLON Lotion.
Even after following the medication course for over a month, there is no change and it's becoming even worse now.
I have enclosed the pictures of my wife's face.
Could you please advise us

Hello. Thank you for writing to us

I have gone through the query and I have also reviewed the Image.
Since she is not breast feeding the baby I suggest that she continues with Codox -L capsules, twice daily. Codox- L is an oral antibiotic i.e Doxycycline and I would suggest her to discontinue Azithromycin, another oral antibiotic. There is no point taking 2 different antibiotics.
I would also suggest that she uses a topical antiacne product and I would particuclarly recommend 'Clindoxyl gel' for her, to be used twice daily for acne.
I would suggest her to stop Nevlon lotion, it is a moisturiser and I don't think she needs it since she already would be having oily skin and therefore acne.
Instead of soap I would suggest that she uses a gentle cleanser for face wash e.g Cetaphil cleansing lotion, twice daily.
She may continue with Cute-E which is Vitamin E but it has no direct role in treatment of acne.

Hi Doctor,

Thanks for the response. I understand She needs to take the below medicines.

1. Codox - L (Antibiotic twice daily)
2. Clindoxyl gel -for acne
3. She is having dry skin and a hence previous doctor advised to use the moisturiser.
4. Cetaphil Cleansing lotion - daily.
5. Cute-E
Could you please advise how long above antibiotics and Vitamin E need to be taken.
Also, Can we know why she is facing abrupt change in face and lot of acne.
How long should we wait for results? please advise if there is a no change what has to be done

Avoid moisturiser

Detailed Answer:
Hi.

I persist with my advice to stop using a moisturiser. I think she is experiencing dryness due to the soap, which is harsh. I have suggested a mild cleanser that does not overdry and if she uses that she may not require a moisturiser.

I suggest her to follow this plan for initial 2 weeks and then assess if she needs further antibiotics. This time period would be required to produce visible results. I am sure clindoxyl would be of great help for her acne with visible results within 2 weeks. She may have to use it longer though and that in turn needs to be assessed after 2 weeks

Acne can be due to hormonal changes, stress, wrong diets, cosmetic use and these factors may also be playing a role in her acne.
