Suggest Treatment For Persistent Wheezing In Children

Question: my son is 17 months old and is suffering form bronchiolitis. He was prescribed 2.5ml histal dm, 2.5ml areoflux, 2.5ml amoxil and 2.5ml predcort for 5 days. He's off the meds one day now but he still wheezing and coughing? what can i do?
Also, he weighs 19lbs, what can i do to help gain weight. He is underweight?

Hello

Wheezing is due to spasm of the airways. Bronchodilator like salbutamol or terbutaline helps in relieving the airway spasm.

Histal DM contains chlorpheniramine(antiallergic) with dextromethorphan(mild cough suppressant). Aeroflux contains ambroxol a mucolytic(helps in thinning of sputum). Amoxil is an antibiotic and predcort is a steroid.

None of these medication contains any bronchodilator.When there is severe wheezing nebulisation with bronchodilator helps a lot.

As already 5 days of oral steroid has been taken,nebulisation with steroid will be helpful(as nebulisation form has minimal side effects).

I would suggest you to start bronchodilator(oral syrup/nebulisation),steroid nebulisation after consulting a pediatrician.

Weight of 19 lbs at 17 month of age is below the lower limit.
I have some queries
1) Was the baby premature?
2) What was the birth weight?
3)Is there any recent loss of weight or he is not gaining weight since the beginning?
4) What diet you are giving to the baby?
5) Is there history of repeated respiratory or intestinal infection?
Answer of all these queries will help in giving answer more scientifically.

Hello

8.5 lbs for a full term baby is very much satisfactory.
In any acute illness there may be recent loss of weight due to change in metabolism and improper nutrition. If properly fed most of the babies regain their weight over a period of time.
Add some oil in the mashed food each time to increase the calorie value.Give him feeds more frequently(every three to four hours).
The incidence of childhood asthma increases if there is a family history of asthma.
Bronchodilators and steroid as and when required along with long term montelukast(to control the asthma) is needed in such babies.

So don't worry.Continue feeding,he will regain the weight. Consult your doctor for the need of montelukast if symptoms are occuring frequently.
