Brief Answer:
Need a rectal exam. continue lactulose.

Detailed Answer:
Hello Dr XXXXXXX
I would advise you to continue lactulose. You should visit a pediatrician or gastroenterologist to get a rectal exam done to confirm then presence of the fissure and to check the presence of a polyp. I suggest to get a TSH exam to rule out hypothyroidism.
In case the constipation persists after a week of lactulose you might need further investigations.
Hope this helps
