Suggest Treatment For Persistent Abdominal Pain And Nausea Caused By Kidney Stones

Question: Hi,
Last to last Thursday I started urinating more often and felt like doing the same, and had mild pain around pubic region. On Friday I saw a doctor and was prescribed cipro 500 mg twice daily for three days.
* On Friday night I got slight pain on the left side of abdomen, the pain came and went. Over next two days I started getting mild pain in both left and right side of abdomen. Pain was worse on lying down and better with standing.Last dose of cipro was monday morning
* On Monday since pain didnt go I went to hospital and my urine was take for exam, along with CBC/ESR and abdomen usg. The doctor asked me to not take cipro till the results came, so monday night i didnt take any medicine.
* On tuesday went back to hospital , urine CUE was normal, CBC with ESR was normal and abdomen ultrasound was also normal. By this time I did not have frequent urination but only pain. Doctor prescribed zenflox 400 twice for 5 days. I took the first dose on tuesday night, however it cause severe pain in my leg and I didnt take the table on wednesday morning. I consulted doctor and was put on cipro for 3 more days.
* saturday morning I took last cipro to complete course. however the pain did not go. I visited the doctor again and he told me to stop medicines and said we will wait and watch. On same day I consulted someone else for second opinion, and the doctor said it looks like cystitis and prescribed me nitroforentine.
* I did not take medicine till monday, I started getting severe back pain which got worse on bending, and better with rest in some position, though the pain in front on left and right side of abdomen went down. Today morning I repeated ultrasound again and two small stones were found in left kidney, 4 and 6 mm each. Also the radiologist diagnosed me with cystitis. I took nitroflourentine first dose tonight,
* I am now confused as how all this was missed in first ultrasound. Also I am having some mild pain in back at right and left side around ribs and abdomen pain on right side is back. I am worried about what can be causing this. Is it something serious like pyelonephritis?
Also I have had mild nausea through out the week after taking cipro so I thought it is due to cipro and didnt think much.
Please advice.

Pain in abdomen and nausea is due presence of stones in kidney .

Thanks for narrating sequence of events in detail about the pain and treatment you were offered ,

At the outset I would state that your symptoms are definitely due to presence of stones detected in kidney and not due to any thing else like Pyelonphritis as suspected by you .

It is unfortunate that the radiologist who did first Ultrasound Scanning failed to detect stones in the kidney .This is should be taken in a light spirit as a human error,

The second Ultrasound Scanning repeated recently has revealed stones of a size of 4-6 mm in both the kidneys .I shall be happy to read the scanning report as regard the hydronephrosis developed due to these stones which is the basic cause for pain in abdomen .precise comments on it .It will help me to make precise comments and suggest further line of treatment .

Since the size of the stones is small we can certainly expect these stones to pass out spontaneously within a week or two without any active intervention ..

Please get your urine culture and sensitivity done to find out organisms causing infection

Please take following treatment
1) Take antibiotics like Cefixime along with Nitrofuarntoin twice daily this will help to eradicate or prevent infection .
2) Tomsulosin once daily .This will help to relax the smooth muscles in ureter and cause dilatation of ureter and there by help in passing the stone down without pain,
3) Tab Drotevarine twice daily this will help to reduce the pain .
4) If you have burning urination take urine Alkalizer thrice daily.
5) Ensure to drink more water this will help to produce more urine and thereby to flush out the stone fast.

I am sure with this treatment the stones will definitely pass out within a week or two and give relief from pain .

Repeat ultrasound scanning after two weeks to find out the statuswhether stones have passed out or not .

Dr, the stones are only in left kidney but the pain is both sides. Also both scans said no hydronephrosis. I am already taking nitrofurantoin twice

The pain in Rt side is called as referred pain

Thanks for follow up .
The pain on Rt side is referred pain .It is similar to as one gets tears from both eyes when one gets irritation of one eye .

Since no pathology like pyelonephritis has been detected on scanning you should not worry at all .
Continue taking Nitrofurantoin for couple of days in addition to medicine suggested in my reply earlier .It acts as urinary antiseptic .
