Suggest Treatment For Penis Infection

Question: Hello doctor,i had sexual contact in januaru 30th.and in may i got peni infection. So one doctor suggest me to do herpes test.so i did test for herpes once in julay and in augest. Can you pls help me

Hi.

I have reviewed the Image. This seems like urticaria.
Urticaria or hives present as skin welts, redness and itching. These episodes are transient i.e last a few hours (4-6 hours) only to return again later.
Urticaria is divided into Acute (duration of urticaria < 6 weeks) and Chronic types (Urticaria for more than 6 weeks). Some patients have it for years.

Acute Urticaria is usually an allergic reaction to an antigen.
The inciting allergen can be either a recent infection e.g common cold Or flu, food or food additives, drugs/ medication (e.g antibitics, pain killers, vitamins), pollen grains etc.
Non-allergic causes of urticaria include drugs like aspirin, NSAIDs (ibuprofen, diclofenac etc), alcohol intake etc.

Patients who are allergic/ atopic Or have a history of eczemas are prone to urticaria/ hives to various environmental allergens.

Chronic urticaria can be due to underlying connective tissue disorder like rheumatoid arthritis, lupus, thyroid abnormalities. These can be suspected from history of patients and confirmed on lab testing.
Physical urticaria like cholinergic urticaria, urticaria to hot/ cold temperature, pressure urticaria, dermographism are usually chronic in nature.

In about 50-60% of urticaria patients there may not be any identifiable cause i.e it is idiopathic.

Treatment of urticaria is symptomatic.
Oral antihistamines e.g cetrizine Or loratadine Or fexofenadine, once or twice daily for a week or 10 days is all that is required in Acute Urticaria patients. Patients with Chronic Urticaria may require longer durations of Oral antihistamines.
Topically you may use a soothing lotion e.g calamine Or an anti-itch lotion.

I suggest a few basic blood tests like complete blood counts, thyroid profile, serum IgE levels, ANA, RA factor.

Macrocytic hyperchromic blood picture; Oral Folic acid supplement

Hi.

I have reviewed the attachments.

The blood indices reveal that you have a macrocytic hyperchromic blood picture i.e raised MCV and raised MCH with low RBC count though your Hemoglobin is good.
This type of blood picture can happen due to Vitamin B 12 Or Folate deficiency Or Alcohol excess.

Do you consume alcohol and how often?

However, since your Vitamin B 12 levels wered not found to be deficient therefore most likely you could be having Folate deficiency i.e Vit B 9 deficiency.
I suggest you to take a folic acid supplement 5 mg once daily for a few days.
Rest of your tests including Thyroid, Urinalysis, ASO, RF factor are within normal limits apart from slightly raised cholesterol.

Alcohol intake could be a reason for your deranged MCV, MCH indices i.e macrocytic hyperchromic blood picture, secondary to folic acid deficiency. I suggest you to supplement your diet with 5mg of folic acid once daily. I will also encourage you to stop taking alcohol.
