There could be a number of causes for such a presentation. He needs to be assessed in an ER setting.

Vitals including BP, Pulse and temperature need to be taken and blood sugar testing should be performed. Before that if hypoglycemia is being suspected than oral/iv glucose should be given immediately. Possible focus of infection should be identified and treated accordingly. Thyroid levels can also be checked at a later stage. Baseline EKG, blood counts and metabolic profile should be obtained to determine the possible cause.
