Suggest Treatment For Painful Constipation In A Child While Recovering From Diarrhea

Question: Hello Doctor ,
My three years old daughter,WT-11.7 kg had diarrhae for a week,I gave her antibiotics flagyl and taxim-o as prescribed by my paediatrician.After that diarrhea stopped in a week. but she strtd to poop after every meal .I gave her aristozyme syrup and zincovit.Gradually this stopped in a week.Now since past three days she is constipated and refuses to sit in the toilet .She says she has got some pain but shez unable to figure out if its pain or burning sensation.Please guide me.

I understand your concern. It is usual for kids to have some constipation while recovering from diarrhea.

I have a few questions for you regarding her constipation and diarrhea. Please get back to me with answers so that I can guide you better.

Questions:
1. Did your kid pass motion or meconium on day one of life?
2. Since how long is the kid constipated? I mean any previous episodes?
3. Does the kid have any bleeding along with hard stools?
4. How much milk does the kid consume per day?
5. Does the kid eat fruits and vegetables (fibre diet) appropriately?
6. Apart from the medicines you have mentioned - did you give any other medicine like Loperamide?

Thanks for your quick response Doctor. Please find my answers below for your questions,

Yes.

2. Since how long is the kid constipated? I mean any previous episodes?

This is the third day of her constipation. Yes, she she is constipating very often. Like once or twice in a month. Also it is kind of mixed, sometimes diarrhea also.

3. Does the kid have any bleeding along with hard stools?

No.

4. How much milk does the kid consume per day?

Two times a day. Total around 300ml for past one month. Earlier she was having around 100ml a day.

5. Does the kid eat fruits and vegetables (fibre diet) appropriately?

She used to have fruits. Apple, Banana. Especially, one banana every night. I was giving Fiber gummies everyday before her diarrhea.

6. Apart from the medicines you have mentioned - did you give any other medicine like Loperamide?

Since the start of the constipation, Gave miralax one cup yesterday evening. Today gave miralax twice, had suppository inserted once, and gave Milk of Magnesium once (she didn't have this completely. She had only little bit of this).

I came to XXXXXXX last month and since then she started having constipation/diarrhea. She used to have constipation earlier in USA but after giving miralax quite often, her motion was regularized. Her bowel movement was fine for 7-8 months. Now she started constipating again after coming back to XXXXXXX She is trying to hold her motion saying it is paining or having burning sensation. I'm really worried on how long she can stay by holding her motion. Will this create any acute problems or any serious situation? Also her food intake is becoming less now. Please advice. Thanks.

Hi....this looks like habitual constipation.

Suggestions:
1. Natural methods are the best to relieve constipation.
2. Constipation is a risk factor for UTI
3. Maximum milk consumption per day should not exceed 300-400ml
4. Minimum 3-4 cups of fruits and vegetables to be consumed per day
5. Toilet training - that is - sitting in XXXXXXX type of lavatory daily at the same time will help a lot.
6. You can give Duphalac 10ml twice a day for 6 weeks...more importantly toilet train her during the period as the Duphalac softens the stool and takes off the fear of painful defaecation.

Thanks for the advice Doctor. She passed motion after that. But I have couple of other questions. Her stomach is bloated since then. Not sure what is the reason. Also she is having foul smell in her mouth. Is this due to some infection or something? Kindly advice. thanks.

Coming to your queries one by one -

1. Bloating of stomach is quite common in constipation. It will get better once constipation gets better.

2. Foul smell in the mouth is due lack of proper brushing. I suggest you do brushing at least twice a day.

3. After night brushing, do not feed her.
