Suggest Treatment For Pain In Mouth And Stiffness

Question: I am 37 years old Male from XXXXXXX I have been suffering from OSMF from last 8 years. I have pain in mouth and stiffness. I can open my mouth up to 40mm but is painful. I can eat normal food. I have have had myself checked with few docs and nobody has seen anything suspicious.

Recently, from one month I have ringing in ears mostly audible in the night. My OSMF also seems to get worse. Sometimes the ringing is loud in one ear. I wanted to know if OSMF can turn someone completely deaf? I have an appointment with ENT in 4 days but want to know if OSMF can make someone totally deaf?

Also, I am based in Australia so guidance on any medicine would be great.

OSMF may cause ETD, not total deafness.

Hi,

Thank you for your query

1. OSMF may cause ETD (Eustachian Tube Dysfunction), but it does not cause total deafness.

2. The ETD may be the cause of tinnitus, if no other cause is found. Get your eardrums examined to rule out common causes such as ear wax buildup. A PTA ( Pure Tone Audiometry ) and I / A ( Impedance Audiometry / Tympanometry ) should be done to evaluate any hearing loss/ tinnitus or ETD.

3. Normal mouth opening is 3 to 3 1/2 vertical fingers. Your mouth opening is slightly reduced.

4. Kenacort (Triamcinolone) injections in the retro molar trigone area will help. Take the injections weekly for 6 weeks on both sides. Use a 10% lignocaine spray before the injections.

5. If the cause has been paan, betel nut or gutkha, stop these items. If not, rule out anemia, vitamin deficiency and autoimmune disorders.

6. Antioxidants will also help reduce the severity of the OSMF. Avoid spicy and very hot foodstuff.

7. Please Upload your test results for discussing further treatment options.

Thank you Doctor. I have upload my recent blood reports. I was a smoker and a Gutkha user for a few years. I have quit the habit more than 8 years back.

My blood shows low MCV and MCH. Can they be related to OSMF?

Also, are you aware of cases were people were successfully treated for OSMF? On the basis that I have stopped using tobacco for last 8 years do you think there are chances that my fibrosis may get worse?

Thank you for writing back.

1. It is good to hear that you have stopped exposure to tobacco and gutkha.

2. Your MCH and MCH values are borderline low as compared to normal. Theses are not related to OSMF. However, you can improve your hemoglobin count with supplements and nutrition.

3. There is no cure for OSMF. It will not increase since you have stopped using tobacco eight years ago. There may be a slight progression due to other factors such as hot, spocy food, but it should be stable.

4. Treatment is done for severe OSMF. Mild cases do not require active management.
