Question: I have a testicle (right) that never totally descended. I am 58 and was told many years ago it is not a problem. Recently the testicle has been aching some of the time.

Its understood that your testis is not in the scrotal sacs but somewhere in the inguinal canal where it has stayed since birth. It is important to get an ultrasound scan of the region to look for any changes in the testis or its blood supply.

Rarely, it is said that a completely undescended testis can develop a malignancy if located in the abdomen. The cause is entirely not known but can be due to temperature differences and hormone secretions. However if your testis is partially undescended then there is no need to worry.

Since you say that you have pulled it away from the body and this caused the pain, I suggest you get an ultrasound scan of testis done to know if there is any inflammation, injury or infection. The blood supply to testis must also be observed to know if pulling it away caused any partial torsion, this is usually accompanied by a swelling and intense pain.

I will try to make this more clear. Both testes are in the scrotal sack. The right testical sits about one half inch higher than the left. Pulling on the testical did NOT create the aching. Sometimes the right testical gets sore from lifting something heavy or straining in some way. It feels like it is sitting higher than normal in the scrotal sack. It is then I will pull on it a little bit to try get it back into its normal position. This situation is different. Never before have I had sensitivity/aching like I have now. In fact,
both testes are sore/sensitive but only SOME of the time. That seems odd to me. I am taking Andro Gel to help alleviate my low testosterone level (low T). Do you suppose that is creating the issue?

Might be rare side effect of Androgel.

Thanks for writing back with an update.

It is possible to experience slight discomfort in both testes and associated increased sensitivity due to application of Androgel. This is a probable side effect of the medication and is among the rare ones. You can discuss this matter with your doctor and stop application of the medication for a few days and observe any relief from pain and sensitivity.

Should the pain persist (more on right side) even after stopping it for some day then it is important to consult the urologist who will examine your testis and then suggest an ultrasound scan to look for conditions like a varicocele or hernia, in which the pain or feeling of testicular fullness might increase with lifting weights.
