Question: Hello, I am 56 years old, and starting to get some hot flashes, and, I think depression. I look okay, I am a little overweight, and I smoke lightly and take alcohol socially. I am afraid to start hrt, because of stroke and mi risk, also I do have some varicose veins do you think this is safe for me? also, having extereme vaginal dryness it drives me nuts. Also, I really can't tell the date of my menopause because I had a uterine ablation in 2004 and have not had menses since, though ovaries are intact. I had a tubal ligation at that time as well. my dear ob/gyn has retired and I do like the one I have now, but my dear ob/gyn always explained things to me . when I asked , " how would I know when I have the menopause, after having my ablation, he said " oh, you will just know"..boy was he right. I am also moody and fear my mouth will get me in trouble. I used to be very mellow. what is going on?

Brief Answer:
Most likely menopause

Detailed Answer:
Hello . welcome to Health Care Forum.
I went through your query and I can tell you this is a very common concern I see in my practice. The symptoms you are experiencing such as mood swings , hot flashes, vaginal dryness , are all commonly seen during menopause. The treatment for alleviating these symptoms is HRT. But in your case as you have told that you have varicose veins it puts you on an increased risk of thrombosis which can lead to pulmonary embolism if the thrombus gets dislodges feom viens. HRT also causes an increase in MI and Stroke but women who have history of cardiac disease are at higher risk. Can you tell me for what condition did you have uterine ablation in 2004? Please also mention what is the duration of your symptoms? Knowing these I will be able to help you more precisely. Regards Dr Manisha Jain.

Yes, I had abnormally heavy periods, and fibroid tumors, I have no other health problems. I do not have high blood pressure or diabetes. When I had my most recent cbc, my primary care provider said I was " better than perfect. No increased cholesterol, triglycerides, etc. as a matter of fact, I have never taken medication and rarely get sick . But I i am not feeling good now and really want to do something about it. I have just been feeling this way within the past 6 months or so not long, but intensely. thank you.

Brief Answer:
simple precautions advised

Detailed Answer:
Good to hear your past history and thus your healthy being.
But menopause is also a part of normal age related changes and thus has to be taken in one's own stride.
You should drink a lot of water and avoid hot and humid climates.
Lessen your intake of caffeine and eat lots of salads and fruits.
Include nuts especially walnuts in your daily diet and increase intake of soy proteins.
Avoid smoking as much as possible.
And for vaginal dryness apply Premarin (local mild estrogen cream so no systemic side effects) cream twice a week for 3 months and then once a week till you lead an active sexual life.
Try these simple measures and see how you feel.
In case still symptoms bother you then start HRT after consulting your present gynecologist.
All the best.
