Question: 57 y/o male with intermittent painful ejaculation and mild dysuria. Yearly PSA 's since 2009 1.6-1.9 last year 1.7. This year 2.0. Family history pos for prostate ca. Had been diagnosed with prostatitis by symptoms only as U/A was neg DRE normal 7 mo ago. Concerned if symptoms c/w Prostate Ca. Ty- XXXXXXX

There is possibility of Cancer of the prostate

Thanks for query .
Based on your symptoms of Painful ejaculation and dysuria are most likely to be due to infection of Prostate (Prostatitis),However it needs to be confirmed by clinical examination by a qualified Urologist .

Consult qualified Urologist for clinical and digital Rectal Examination and get following basic tests done to confirm the diagnosis .
1) Urine routine and Culture
2) Semen examination and semen culture
3) Ultrasound Scanning of abdomen
4) Swab test and culture of a urethral discharge obtained after prostatic massage .

Please note Prostatitis takes long time to get cured hence it is mandatory to confirm the diagnosis and take treatment for 4-6 weeks .

In view of your PSA consistently being within normal range (below 4) There is no possibility of cancer of prostate ,

In the mean while start taking antibiotic like Doxicycline twice daily with anti inflammatory medication like Diclofenc twice daily .

Further treatment will depend upon the results of these tests and final diagnosis .

Ultrasound was normal- there are times when i have some rectal discomfort elieved by BM- my free PSA was 43.5%

Thanks for follow up .In view of normal PSA and absence of any predominant urinary symptoms there is no possibility of any major issue .

Get relaxed and try to allay your anxiety .Take medications suggested for 10 days

I am nervous because my father had prostate CA - also i forgot to tell you the pain i feel is sharp in the head and shaft of the penis and urination is mildly painful- also if i have increased sex the pain is worse for about 24-48 hours- thx again and i appreciate your patience

Thanks for follow .Though hereditary plays role in getting many diseases in next generation and your father had Ca prostate that does not mean that you are likely to get the same .

Since your PSA which is basic parameter in screening of cancer pf prostate is well within normal range you should not be worried about you getting cancer at all at this stage of life .

Try to allay your anxiety and repeat your PSA levels frequently at the interval of every six months .This will help you to be free from unnecessary mental agony .
