Brief Answer:
need to know few things.

Detailed Answer:
Hello dear, thanks for your question on HCM.
I can understand your situation and problem.
I want to know few things first.
1. Are you smoker?
2. Are you a known case of cancer?
3. What symptoms do you have?
There are many variations present normally in lobes of lung in humans. They can be normal incidences too. Extra lobes or fused lobes or accessory lobes, accessory fissure etcare normal anatomical variant s of lung. It is not a disease process. It is normal anatomical variation. So subpleural lobe is not an organ it is mostly accessory lobe.
Metastatic nodule suggests you are having primary cancer somewhere else. If this is the case than your primary cancer is staged as stage 4 disease and it becomes inoperable, only palliative care should be done in the dorm of chemotherapy and radiotherapy.
So, let me know answers of above asked questions, so that I can guide you further.
I will be happy to help you further.
Wish you good health.
Thanks.

Yes my dad had colon cancer surgery in march last year . Recent ct / pet showed a liver met , a aortocaval node . Started irinotecan .

January scan showed a nodule in the sub pleural lobe .

How come the radiologist did not state the measurements of the nodule ?

Brief Answer:
you have mentioned size of nodule in 1 st query.

Detailed Answer:
Hello dear, thanks for your follow up question.
You have mentioned the size of his metastatic nodule in lung in the first query that it is under 1 cm.
This must be the size of the nodule.
And he needs chemotherapy and radiotherapy after consultation of oncologist.
Hope I have solved your query.
I will be happy to help you further.
Wishing good health to your father.
Thanks.

The chemo irinotecan and xeloda have not worked though and they can't give more . What should we do? Can the 3 mets be ablated?
doctor
Answered by Dr. Drkaushal85 (11 minutes later)
Brief Answer:
radiotherapy is an option.

Detailed Answer:
Hello dear, thanks for your follow up question.
It is quite possible to have resistance to chemotherapy drugs.
So radiotherapy can be the option in such cases.
But again risk-benefit ration after giving radiotherapy should be discussed in detail with Radiotherapist.
And surgery of metastasis (metastesectomy) can be done. But the risk of surgery and post operative complications are very high. So again you need to discuss risk-benefit ratio in detail with oncosurgeon.
Hope I have solved your queries.

Thx . What kind of radiotherapy? Would radiotherapy help prevent mets ?

Can the three mets b ablated ? There's 1 on sub pleural, 1 in liver and 1 aortocaval node. All small .

Plz can u finally also advise re survival length ? Can he still have a few years or not

Answered by Dr. Drkaushal85 (36 minutes later)
Brief Answer:
yes, radiotherapy will clear the mets.

Detailed Answer:
Thanks for your follow up question on HCM.
There is targeted beam radiotherapy available. This targets the metastatic nodule and destroy it.
It is specifically given in isolated metastasis.
Radiotherapist mostly prefer cobalt for this.
So better to consult radiotherapist or oncologist for this therapy.
And if his primary tumour is under control than these small metastases should not be operated because operation is itself very challenging and outcomes are poor. Better to go for radiotherapy.
And prognosis is good if he is having only these 3 small mets. Five year survival rate will be 50% and life expectancy is almost 2-3 years.
So he is still having years to live, so consult oncologist and start appropriate treatment.
Hope I have solved your query.

For nano knife should
We go for it then?

Is there any way to control the micro mets hidden in the body ?
doctor
Answered by Dr. Drkaushal85 (14 minutes later)
Brief Answer:
definitely go for nano knife.

Detailed Answer:
Hello dear, thanks for your follow up query.
Yes, you should definitely go for nano knife. It is a too option.
Systemic chemotherapy is the only option to prevent micro metastases in the body.
So better to consult oncologist and start chemotherapy other than xeloda and irinotecan.
Oncologist will guide you more in chemotherapy aspect.
Hope I have solved your query.
