Suggest Treatment For Low BP And Dizziness

Question: The patient is having dizziness and weakness due to medication. Her blood perssure is 90/80. What medication need to be titrate. We think is divonan 80. Instead of once can be reduced to 40 or 20. Thank you Cardic doctor please.

Hello,

when prescribing antihypertensive treatment, the blood pressure goal (except in very special conditions) is to have a systolic blood pressure between 130 and 140 mmHg. Such a low blood pressure (90mmHg) is a sign for lowering the intensity of treatment. If 90mmHg is only an isolated low measurement then this is not a real problem. If many measurements are at this level though, then a change in the regimen is recommended.

Of course your best choice is to ask her doctor for an opionion about her situation but I'll tell you what I think.
In cases like this I usually start the de-escalation with diovan. A trial without diovan will show whether this was enough or inappropriate. Taking into account the potency of the drug, I believe that stopping it won't cause any problems to the patient and the blood pressure would be close to the target level.
