Suggest Treatment For Lethargy And Kidney Problems

Posted on Fri, 24 Oct 2014
Question: I have been feeling really tired lately. I've also had a bad head ache almost every day, been very bloated, and got a weird rash that was incredibly itchy and went from joint to joint (first elbows, then knees, etc.). On top of that, I got kidney stones and was so sick I was vomiting blood. Nobody can figure out what's behind all this. But I had blood taken before the kidney stones and then again while I was in the hospital with them. Three times in a row, it showed low MPV. It also showed high CH50. After being in the hospital, I had low eGFR, high Lipase, low Magnesium, high Neutrophils and low Lymphocytes, but some of that may just have been because of what was happening at that moment. This things were either not tested or were normal when I had blood drawn the week before.

Any suggestions?

Brief Answer:
Immunologic kidney injury.

Detailed Answer:
Hi Madam

I have carefully read your query and understand your concern.
The main crux in your health issue is low eGFR.This revolves around kidney shutdown secondary to renal stones complicated with superimposed urinary tract infection reflected as high neutrophils, high lipase and vomiting sensation.

Acute kidney injury ,more appropriately called is also associated with serum electrolyte disturbances like low magnesium.

This kidney injury in your case has additional component of immunologic injury manifested in the form of elevated CH 50 and low MPV.

The migratory rash is could be due to low platelet count manifesting as minor skin bleeds, associated with itching due to immunologic component.

I advise to have complete autoimmune antibody panel with a renal biopsy to accurately document the disease component and pave way for accurate treatment guidelines.
