Suggest Treatment For Left Shoulder Pain

Question: Hello doctor,

My mother age is 74, and from 3 days she is experiencing left side cheast pain with heavyness also she says that her left arm is paining as well. She also has oesteoprosis but the pain in left hand also started along with the chest pain 2-3 days back. We consulted a local cardiologist and she underwent ECG and 2d echo, reports are attached.

Local doctor said reports are normal and prescribed these medicines

1.Tricinod
2. Dompan
3. Clopitab
4. Lipicure

my main concern is her chest heavyness and pain which corelates with left shoulder hand pain, these symptoms gives an implication of heart attack.

Please review and suggest best course of action immediately.

Her Echo and ECG reports are alright

Detailed Answer:
Hello

After reading the details provided by you of your mother's symptoms and after reviewing the reports and her medical history of cholelithiasis (gall bladder stone) and other comorbidities I understand your concern.

Her ECG is perfectly normal and also her echocardiography shows normal LV function. This makes the presence of heart disease a bit unlikely however her symptoms are quite specific for cardiac issue.

So in my opinion she should go for a stress test which could be done in form of TMT or stress echo. I think stress echo will be a better option for her considering her age, sciatica and spondylosis. This test will help unmask any underlying coronary artery disease. If that comes out to be positive then a ANGIOGRAPHY needs to be done.

Till then you can continue with the prescribed medicines. It's advisable to get her evaluated in a good cardiac center as soon possible.
