Suggest Treatment For Itching And Burning Sensation During Urination

Question: Had sex with unknown lady about 15 days back, from third day onwards I felt itching in the entire body lightly and burning sensation while urination. No bubbles or rashes found on penis. Approached doctor after 8 days and got checked for blood tests for HSV.1 & 2 tests and vdrl. Got report after 3 days and tests were negitive and normal.

I have burning sensation at penis while urination, doc suggested to use GLEVO 750 mg tablet daily at night. Iam using this tablet from last six days but there is no improvement and still have pain at penis.

please suggest.

Hi.

Unprotected sex is risk factor for transmission of various STDs.

Burning sensation on urination and the timeline of symptoms i.e 3 days after unprotected sex suggests that it is likely to be due to urethritis.

Urethritis after unprotected sex could be due to Gonorrhea, Chlamydia, Trichomoniasis and/or Mycoplasma genitalium.

You have been taking Levofloxacin tablet. Though Levofloxacin is effective against both gonorrhea, chlamydia and mycoplasma genitalium but it is not a recommended Or first choice treatment for these and there might be substantial treatment failure rates with levofloxacin.

If I was the treating doctor I would suggest Re-treatment with recommended drugs i.e Azithromycin and Cefixime, to cover for all these possibilities.

I would also suggest treatment for trichomoniasis with metronidazole.

Rarely genital herpes can also cause intraurethral lesions and it can also be a reason for urethral burning.

Tests for genital herpes just 8 days after a risky sexual encounter is too early and may be false negative.
I would suggest that you repeat this test at 4 weeks to hold some diagnostic value.

You must also get screened for HIV, VDRL at 4 weeks.

Thanks a lot for your reply. I never did sex with others, and this happened unfortunately.

Yesterday night I talked to my Doctor and explained about the burning in urination.

He suggested to use 'Urispas tablet' daily Three times and take syrup 'Citralka' for Three times, and to continue 'Glevo 750' for another Two days, and asked me to meet him on Thursday.

Sir, when I use Glevo 750 in night time, I am finding burning sensation on the entire body especially in mid night. Further today I found some burning sensation at anus and got loose motion, and I think this may because of using Antiboitics from the last 7 days.

Sir, you suggested to start re-treatment with Azithromycin & Cefixime and also suggested to take 'metronidazole'.

Please tell me daily how many times I have to take these Tablets/Timing/mg and for how long. Please also suggest me, should I continue 'Uripas tablet' & Citralka tonic'. From when should I start these medicines.

Sir, kindly tell me will there be side effects by taking these many Antibiotic tablets. Already I am feeling unwell with Glevo 750.

As advised, I'll get suggested tests after 4 weeks.

Hi.

You can continue with Urispas and Citralka as they are urinary analgesics and would help with pain and burning sensation on urination.

For Urethritis, Azithromycin is given as a single dose of 1 gm and it covers for chlamydia, mycoplasma and ureaplasma urealyticum whereas Cefixime is also given as a single dose of 400 mg and it covers for N. gonorrhoeae (Gonorrhea).
Metronidazole is also given as a single 2 gm dose and it covers for trichomoniasis.
There can be minor side effects like looseness of stools with azithromycin and minor side effects like metallic taste, dizziness etc with metronidazole.

Alcohol should be strictly avoided along with metronidazole.

All these are recommended first line drugs for suspected urethritis according to CDC XXXXXXX and WHO/ NACO (India).

These are prescription medications and therefore I suggest that you take these only under supervision and approval of your doctor.

I am very much Thankful for your reply.

I used Glevo 750 for seven days. As I was feeling burning sensation over body by using Glevo 750, I stopped taking this tablet for Two days. I used Urispas tablet and citralca syrup only for 2 days and found not much burning sensation while passing urine. However, I had burning sensation in Eyes and felt feverish.

I conveyed the above symptoms to my Doctor yesterday.

I have also discussed & shown your suggestions to my Doctor.

But, he advised me to continue Glevo 750 for another three days and Urispas tablet and citralca syrup for another Five days.

Sir, yesterday night I have taken Glevo 750 and Urispas/Citralca. But, whenever I take Glevo 750 I finding burning sensation over the entire body (this I explained all the time to my Doctor, but he is not accepting this) and advising to use it.

Now, I am feeling mild burning sensation while urination (may be because of heat developed in the body due to Antibiotic tablet/Glevo), but no marks on the penis. (I rubbed my penis yesterday and I found my penis much red in color and I told this to my doctor and he said when we rub it normaly happens red and nothing to worry).

Sir, now I have burning sensation in Eyes and finding my eyes very dull and light reddish and feeling feverish.

It has passed 3 weeks. Please tell me cannot I get tested for knowing any STD decease and take the right medicine to cure it.

Sir, I am totally confused/disturbed.

Please suggest and advise me what shall I do.

Will there be anything wrong (any health problems arise) if I take the medicines suggested by you. You have suggested only single dose of three medicines.

Please give me detailed reply Sir.

Hi.

My advice would be to wait and watch for another 2 days and if the burning sensation on passing urine does not improve you may ask your doctor to reconsider.

In my opinion it is always better to treat with first line drugs rather than with alternative drugs, more so if the patient has not responded to alternative drugs.

Besides, I would have also liked to treat you with metronidazole in addition, to cover for trichomoniasis. This part was anyways not covered by levofloxacin.

I suggest a swab test from urethra for gonorrhea and chlamydia which would be helpful. Apart from that you must get yourself screened for HIV and VDRL at 4 weeks.
