Posted on Fri, 16 Oct 2015
Question: Dear Doctor,
My 3.3 years daughter having high fever closed to 103-104 since last 3 days. I visited a doctor nearby and prescribed some medicines with antibiotic. Unfortunately, the temperature does not go down to normal until and unless providing the flexon which was also prescribed for high fever. I therefore have some queries regarding it.
1. How safe in providing flexon?
2. How often Can I provide this medicine to her
3. What may the causes of this high fever
4. Any home-remedy for reducing the high fever?

Brief Answer:
I would recommend a few lab tests

Detailed Answer:
Hi
Welcome to the HCM

I completely understand your concerns. Most common reason for fever in this age group is self limiting viral illness. They just need symptomatic management with antipyretics( fever reducing medicines) and they usually start improving in a couple of days.

But since your child's fever is high grade and it's not improving even after 3 days, so I would recommend a few laboratory tests to rule out any bacterial infections:
1. Complete blood count and peripheral smear examination
2. C reactive proteins
3. Throat swab culture in case she has any throat congestion to rule out streptococcal infection
4. Urine routine and microscopic examination along with culture.
5. Chest X Ray if has cough or breathing problems

You may upload the reports for my reference.
For the time being you may continue flexon as advised but do not give more than 3 times a day.
It's safe in the treatment doses for fever if given for a few days.

The cause of fever may be:
- Viral infections
- Urinary tract infection
- occult bacterial infection

You may give tepid sponging along with flexon for fever spikes.
In case she doesn't respond, Syp. Mefenamic acid may be given.

Don't worry, she will be fine.
Take care

Dear Doctor,

Thank you so much for your kind reply. Here is submission: After giving the following medicines which were prescribed by the earlier doctor, her temperature goes down to a normal and she is responding.
1. Cefiwork- 5 ml -two times a day
2. Wikoryl DS-5 ml-three times a day
3. Bestozymes- 5ml- two times a day

I stopped giving her the flexon and only given two times two day before yesterday. However, she is also eating and feeling good as far as fever is concern right now.

As the temperature gone down, we haven't tested the blood as not prescribed by the doctor . Kindly do tell if there is a need.
But we noticed a new problem of having mild rash in her face and body. What should we do? One more day, we need to provide the above medicines.

Kindly suggest us so that we can take appropriate measurement.

Brief Answer:
No need for tests if child improving

Detailed Answer:
Hi
Since your child is improving, so there is no need for any testing. You may continue the same medications. Also, hold bestozyme as it might be the reason for rash. Apply lactocalamine lotion over rash.

In rare case, if the rash worsens, then send it's complete information regarding location, appearance along with pictures for my reference.

Take care
