Brief Answer:
Call the ambulance

Detailed Answer:
Hi
Ihave read your query
I think you should definitely call the ambulance.
In his case it may well be a cardiac attack or a heart attack in simple language.
In this case time is crucial.
The faster he gets to a hospital that has a cardiologist and that is equipped with facilities to treat a heart attack, the better.
SO I THINK IT WOULD BE BETTER TO CALL FOR AN AMBULANCE, IF POSSIBLE WITH PARAMEDICS.
I understand I am not explaining too much, or giving too many reasons. But as I said, time is of essence.
Best of luck
