Question: I was diagnosed with SVT from my holter monintor I wore for 30 days. They decided to do a EP study and did not find anything until they used isoproterenol at which time they stated i had a episode of atrial fib. They began treatment right away for afib. I decided to get a second opinion and my new doctor said that a isoproterenol induced afib attack does not mean that you have afib and I should not be getting treated based on 1 episode that occured due to isoproterenol induced afib. I am more confused than ever and would like to know what's true and what's false. Do I have afib based off one episode or can anyone have a afib episode after taking isoproterenol? Please help btw I am 27 white female 115lbs 5'5 average blood pressure 110/60 average heart rate 84.

Dear Ms,

Before I answer your query, I have some questions for you.
1. What was frequency of your SVTs?
2. Could you, please, attach your holter report?
3. Have you been treated for SVTs? If yes, how was the efficiency?
4. For how long have you had SVTs

Regards,

I only have one image. They said my heart was 150-179 when i have episodes. I was treated twice both times with Metoprolol twice a day. I was treated with sotolol after having been diagnosed with afib 80mg twice a day. both medications made me extremely fatigue. however I did not have any heart racing issues. They first diagnosed svt in 2012 while I was pregnant. I can hear my heart beat in my ears sometimes when I have bad episodes. I have never had a irregular ekg ( ive had about 15 total over the years). I have a episode maybe a few times per week. I am curious if I am having exteme anxiety attacks and not SVT at all. But it bothers me that I was told I have Afib and now a new doctor said I do not have Afib just because it was medically induced.

Brief Answer:
Atrial fibrillation is less lkely

Detailed Answer:
Dear Mrs,

Unfortunately the provided image quality doesn't allow determine type of SVT.
Anyway it doesn't look like afib.
Your case is one of difficult situations that arrhythmologists face, when patients has palpitation, episodes of tachycardia, but there is nothing during EP. Actually your second doctor is somehow right: sometimes isoproterenol can induce arrhythmias, which patient doesn't have.
On the other hand you have tachycardia few times a week which is quite frequent. So you will need treatment. I would advise you to test your thyroid, as thyroid disorders can cause such symptoms. You can consult psychiatrist if you suspect anxiety.
If there will be no thyroid disorder or anxiety you can try to go with low dose beta blocker from newer generation which have less side effects such as fatigue.

Hope I could help you
