Brief Answer:
It is a Haemorrhoid.

Detailed Answer:
Hi
I saw the pic.
It is a haemorrhoid.
It is not a Fistula as there is no external opening.
If there is a blood clot in the haemorrhoid (Thrombosed haemorrhoid ,it will be painful.
I prefer to do surgery and remove the swelling.
Please consult a Surgeon, get examined clinically and proceed accordingly.
You should take high fiber diet.Avoid Non vegetarian food.
Drink plenty of liquids.
Your stool should be soft.
Nothing to worry.
Hope I answered your query. If you have any questions, I will clarify further.

Brief Answer:
It is not a Fistula.

Detailed Answer:
Hi
As per your pic,it is not a Fistula.
Fistula has an external opening from which pus/blood comes out and wet the undergarments.
It looks like a Haemorrhoid only.
Please consult a Specialist, get examined clinically and proceed accordingly.
