Question: Hello sir,My age is 25. Height 185cm and weight 76kg. Sir my problem is, my chest is looking like a boobs. Can you please guide on this, what will i do next.

You seem to have gynecomastia. As such you don't seem to have any weight issues. Your weight is appropriate for your height.

I would like to gather few more details from you.

-How long standing is the problem? Is it since puberty? Is there a family history?
-Were you overweight in the past and had lost weight?
-Is there any tenderness in this area/around the nipple?
-Is there any discharge from the nipple?
-Is it symmetrical on both sides or is it asymmetrical?
-Can you upload a picture, so that I can have a better idea?

From last 8 years. It get started at the age of 16.

-Were you overweight in the past and had lost weight?
No, before my weight is very less, 2 years back i gained the weight.

-Is there any tenderness in this area/around the nipple?
No sir, i didnot feel any like that issues.

-Is there any discharge from the nipple?
Nothing

-Is it symmetrical on both sides or is it asymmetrical?
yeah almost symmetric. Right one looks little big.

-Can you upload a picture, so that I can have a better idea?
At present am in office, tomorrow sure i will upload the picture.

Persistant pubertal gynecomastia

Hi.

You seem to have a long standing issue (8 years). I would keep a possibility of Persistent pubertal gynecomastia in your case.

Persistent pubertal is one of the leading causes of gynecomastia.

The breast consists of two main components:
-Glandular tissue - which is firm and dense.
-Fatty tissue - which is soft.

Gynecomastia is thought to be caused by an altered ratio of estrogens to androgens, an increase in estrogen production, a decrease in androgen production, or a combination of these factors increase the size of male breast tissue. This can occur because of expected hormonal changes during puberty. The condition often goes away on its own, but may persist in some.

There is a definite hereditary component in persistent pubertal gynecomastia. A family history of gynecomastia has been elicited in 58% of patients with persistent pubertal gynecomastia.

Obesity can also be associated with increased peripheral conversion of androgens to estrogens and is associated with a higher prevalence of gynecomastia, as you must have noticed that obese people more often than not have gynecomastia.

The obesity connection also comes into play in the second type of gynecomastia, referred to as pseudo-gynecomastia, which are formed by fat tissue layered over normal-sized male mammary glands.

However, obesity related gynecomastia is not an issue in your case.

Hi.

I have seen the pictures. The problem is not that severe.

You could benefit from change in lifestyle habits and dietary modification. Regular exercise and weight control regime might prove to be beneficial.

Medical treatment for gynecomastia have included Tamoxifen, Clomiphene citrate, Danazol etc but they are more effective for recent onset gynecomastia rather than for a long standing problem

If this proves ineffective you may opt for Reduction mammoplasty/surgery. As the ratio of glandular to fatty tissue varies from patient to patient. In individuals with gynecomastia there may be either too much of fat or too much of glandular tissue or both types of tissue. Liposuction would remove excess fat and Excision can be done for excess glandular tissue. A surgeon can decide on examination whether there is an excess of glandular tissue or excess of fatty tissue Or he may perform a mammogram to confirm.
