Brief Answer:
About gynecomastia:

Detailed Answer:
Hello and thank you for asking!
I have gone carefully through your concern.
You are right, theree many causes of gynecomastia.
Elderly men between 50 to 80 years of age are considered a high risk group due to low testosterone levels as a result of advancing age.
This is the most common cause at your age ( of course if you are not taking medications that cause gynecomastia such as; spironolactone, corticosteroids ect.).
To verify it you should check total testosterone level and SHBG (testosterone binding proteins).
The thyroid function should be checked also, an over active thyroid would be another cause of gynecomastia.
Alcoohol and smoking is suggested to be withdrawn in these cases.
Liver and renal function assessment is necessary too.
Of course a clinical examination is very important to evaluate the gynecomastia and rule out breast cancer that in fact is rare ( 1% of cases).
If you have done any test please do not hesitate to attach it. Will be a pleasure to help you further.
Kind Regards,
Dr.Mirjeta

.thk u 4 your quick respone. im on pd dialysis for 9 months now. im also showing a hi psa of 24. for reason of any more complications, i've choosing not 2 to have a biopsy of my postrate. my life has been spared for a few more years with dialysis.
i had a mammography last week. the finders were : flame-shaped retroareolar opacity seen on the left breast s unremarkable. no suspicious mass, constipation, architectural distortions. i can feel a hard mass under nipple, slightly sensitive.
if i do nothing, how will the mass evolve .i also have hep c, since 1987. last liver panel was in 2015 and all was well with a critter count of 700000. thats my complete history. my question is : over the next 2 years how much will the mass effect me.
thank you again , its nice getting to talk with some one on a peer level.

Brief Answer:
Check CA15-3

Detailed Answer:
Hello again,
Sorry to learn about difficult situation.
It is difficult to talk about the prognosis of this hard mass that you palpate and flame-shaped opacity in mammography, without a cytology report.
If you refuse a cytology of the breast mass too, i would recommend at least to check the tumor marker; CA15-3.
It can give further information regarding to the nature of the mass.

Please feel free to ask if you have further queries.
Regards,
Dr.Mirjeta
