Suggest Treatment For Fungal Infection In An Infant?

Question: My daughter is 21 months old, from past few days i am observing paper put between her vagina and anus (pic is attached). She wears Mamy Poco Pants (Pull Ups) and we apply rash cream in night before she sleeps and in day we apply baby powder at every diaper change. We have applied Antibiotic and pain relief cream (Bacitracin zinc, Neomycin sulpate, polymyxin b sulfate and pramoxine hcl) cream for 3 days now and no improvement is observed presently. Currently we are travelling so we dont have access to doctor physical visit. Please let me know which OTC cream i can use so that she an be cured

Dear Mr XXXXXXX

There is excoriation(raw skin) between vagina and anus. It is likely to be due to fungal infection.

I would suggest the following

1. Give Diaper free interval for 60-90 minutes in a day.

2. Gently clean the area with baby wipes especially Himalaya Baby wipes if available at time of changing diapers.

3. keep area clean and dry

4. Local application of combination of Betamethasone(steroid) and Clotrimazole(anti fungal) applied thrice a day will be helpful

5. Night time application of Dr XXXXXXX Zinc oxide ointment.

Do keep me updated about progress.

Thanks for prompt response. We normally wash her lower part with every diaper change with water and mild soap. Just wanted to check if we should continue with it or just use wipe till fungal infection is cured?

Hi Doctor

My wife just updated me that my daughter had constipation recently. would this cause such infection to arise?

I am bit skeptical to use only wipes to clean her potty, as that would still leave it unclean. Is it fine for me to use water and mild soap (johnson baby shampoo) to clean her after poty instead if just wipe? is there any harm in using water and mid shampoo for cleaning her up? can water cause fungal infection to spread or increase? Please elaborate?

Hi
You can use water and mild detergent. Excessive moisture and wetness is one of the predisposing factor for fungal infection. Cleaning with water will not cause fungal infection. But after cleaning it is important to dry the area by dabbing it gently with soft cloth.
Rest be assure there is no cause for concern, you can follow whatever is convenient for you.
