Suggest Treatment For Frozen Shoulder

Question: Hello Doctor

My mother is 66 and is having pain in the highlighted section. Pleas see the pictures. She had Typhoid on 23-28th August since then the pain was there in the whole body which has subsides now the pain is in the highlighted section only. She is finding it difficult to wear sleeves sweater, if she tries to carry something then there is pain even in the hand. It is more in the right side of the body, very less on the left side. What do you suggest?

Does she have pain and restriction of movement on lifting the shoulder and arm overhead or lifting weights?

What are her usual fasting and post meal blood sugar levels ?

Any pain radiating from the back of the neck to the hand?

Any tingling or numbness?

Has she ever been diagnosed with Frozen shoulder ?

If she has radiating pain from the back of the neck to the hand, with tingling and numbness, it could be cervical spondylosis.

According to the area shown in the photo, it appears to be a case of shoulder joint adhesive capsulitis or Frozen shoulder.

It would be helpful to get an Ultrasound or MRI of the affected shoulder.

Apply hot packs to the affected region. Perform range of movement exercises of the shoulder. Avoid bending neck forward or lifting weights. Avoid using thick pillow.

She can take Tablet Ibuprofen 400mg twice a day after meals and Tablet Rabeprazole twice a day before meals for 5 days.

What are her usual fasting and post meal blood sugar levels ? Don’t know the levels, but her H1bAc is normal since last 24 months (Ranges 5.9-6.1), Last one is 5.9

Any pain radiating from the back of the neck to the hand? No

Any tingling or numbness? No

Has she ever been diagnosed with Frozen shoulder ? No

If she has radiating pain from the back of the neck to the hand, with tingling and numbness, it could be cervical spondylosis. Radiating Pain is not there.

According to the area shown in the photo, it appears to be a case of shoulder joint adhesive capsulitis or Frozen shoulder.

It would be helpful to get an Ultrasound or MRI of the affected shoulder. Can you suggest the name of the Ultrasound or MRI for the exact region so that I can get that done?

Do get USG or MRI of the affected shoulder to look for adhesive capsulitis. Also an Xray AP and Axial views of the affected shoulder will be helpful.

Find attached the x-ray report. Please suggest if USG Right Shoulder is still necessary. Hot fermentation have started. Is it still recommended to start the below:

Tablet Ibuprofen 400mg twice a day after meals
Tablet Rabeprazole twice a day before meals

5 days.

There is no shoulder arthritis.

USG is needed to confirm Frozen shoulder which cannot be seen in an Xray.

Do take Ibuprofen and Rabeprazole for 5 days.

There is some tear of muscles around the shoulder as well as possibility of Adhesive capsulitis.

The MRI will confirm the presence of any tear and also help to decide the treatment and exercises.
