Suggest Treatment For Fever, Diarrhoea And Vomiting In A Child

Question: My daughter is aged 14 months..(10 kg). She is having fever for nearly 4 days..she was having temperature and continuous loose stools and vomiting. Temp didn't come down even after giving her paracetamol.. Now after blood tests she is detected having Malaria... Doctor has prescribed her Lariago syrup..2 tea spoons first time first day and 1 tea spoon after 6 hours the first day..then following 3 days 1 tea spoon in the morning alone..Dear sir..shd I continue paracetamol also and is the dosage given above OK..she is having loose stools continuously..what shall I give her?..how many days will it take to recover from malaria..pls advise

Welcome to the HCM

For malaria treatment, you may continue lariago as advised. Give 10-5-5-5 as advised. No need for 4th day dose.

Also, consider giving primaquine after consulting your doctor as per malaria type for preventing relapses. Plasmodium falciparum needs primaquine 0.75 mg/kg single dose but Plasmodium Vivax needs 0.25 mg/kg/day for 14 days. Before giving it, G6PD test is required.

Never give lariago empty stomach. If child vomit the dose within 45 minutes, then repeat the same dose. If child is febrile, then reduce fever by crocin before giving lariago.

You may give Racecadotril granules (10mg) thrice daily for treatment of diarrhoea along with regular feeds and oral rehydration therapy. If the child is not nauseous, give zinc supplement such as Syp ZnD or Zinconia (5ml once daily for14 days) for early recovery and further immunity against similar episodes.

Hopefully he will be fine with in few days.

Dear Mam, I missed out giving 10 ml and gave only 5 ml for the first dosage and I have given the second and third dosage of 5 ml each..will there be any problem for reducing the first dosage of Lariago as 5 ml instead of 10 ml..is there any alternative.. Pls advice
.how many days will it take for the temperature to reduce after starting this treatment with lariago..

Do not worry. Give him 5 ml extra dose 12 hours after the next dose. That is 5-5-5-5-5 instead of 10-5-5-5.

The fever usually comes down by 3rd day of lariago. If it doesn't, then recheck for malaria titres and rule out other infections such as typhoid, urine infection, chest infections.

Hopefully he will be fine.

Fever has come down.. One dose of lariago is due , now the baby is continuously vomiting even for water and not taking any food and she is also having loose stools (very watery) more than 12 times a day. While vomiting it seems to be with mucous...baby is feeling very week..please adv on the medicines..

Repeated vomiting can be due to gastritis associated with disease or lariago. Don't worry. Give him Syp. Emeset (2.5 ml thrice daily) along with Syp. Digene (2.5 ml thrice daily). Hopefully he will be fine. In case, he doesn't respond and look sick then visit pediatrician for repeat clinical examination and laboratory evaluation along with injectable therapy.
