Suggest Treatment For Fever And Breathlessness After Being Treated With Stents

Question: Dear Doctor,

My brother in law has been treated with 7 stents - one in LAD , 2 in OM1, 2 more in IM , and 2 in RV/RCA junction on 26th and 27th Dec 2014. He started recovering , showing fever 101.5 average , some breathlessness but was recovering and discharged.

However, last night he had one seizure at home and was rushed to hospital where he had another seizure. Now he is talking . They did Cat Scan and was clear. IM doctor examined him and found every thing clear.

Please guide. Thank you.

I am Dr.Ajay Panwar, a neurologist,here to answer your query.

Once structural causes for a seziure at this age(>35 years) are ruled out by a magnetic resonance imaging(MRI) or computerised tomographic scan(CT scan) of the Brain[MRI is more sensitive than CT in seizure patients],the most common etiology to look for is metabolic.
He needs to be evaluated for the following metabolic parameters-
1)Liver enzymes
2)Kidney function tests(Blood urea and serum creatinine)
3)Serum electrolytes
4)Blood sugar

Another important thing to note here is that considering his cardiac profile,he is at high risk for stroke.Seizure can be the first manifestation of stroke, often.If a CT scan is done in the initial 24 hours of a stroke,it may be nagative.MRI is more sensitive to detect early stroke or,a repeat CT should be done.This should be given a consideration.
