Question: Hi Doctor,
I am XXXXX 28 YEARS old. I am suffering from shallow breathing(Improper Rhythm, Sometime Pause in the breathing) during night time especially during sleeping accompanied with Gastric problem from last month and during day time I am having a lump/congested feeling . I had consulted the cardiologist and he had taken all the test related heart(ECG, Tread mill, Echo) and confirmed my heart is normal condition and he also prescribed the medicine Takeform(Prior to Break fast) for the Gastric problem. I also had consultation with pulmonologist last week for the respiratory problem and he conducted breathing exercises and ensured the lungs functioning are good and suggested me the cause may be due allergy or Acid reflux and he also prescribed me a inhaler ( 2 puffs morning and night) for 30 days. I am currently using the above mentioned medicines(Inhaler-Started using 2 days and Takeform- Stated using 2 weeks). Now the Gastric problem got reduced , but the breathing problem still prevails. Kindly clarify me this breathing problem will be subsidized on due course or I have to consult with ENT or other specialist for the breathing /Respiratory problem, since I am also having Tonsils and dust allergy problem.

Brief Answer:
You should definitely consult ENT doctor.

Detailed Answer:
Thanks for your question on Healthcare Magic.
I can understand your concern.
I have gone through the reports you have attached.
Your all blood reports are normal.
Your all cardiac investigations are also normal.
Your lung functions are also normal.
So I don't think your breathing problem will be solved by current medicines.
We should definitely rule out ENT cause for your symptoms.
So consult ENT doctor and get done endoscopic examination of nose to rule out polyp or ant other obstructive pathology.
If this is also normal then probably stress and anxiety are the causes for your symptoms.
So please let me know
1. Are you having stressful life?
Please reply me answer of above asked question, so that I can guide you better. I will be happy to help you further. Wish you good health. Thanks.

Hai Dr XXXXXXX
Thanks for your prompt reply. As asked in your response,I am having some stress in my official work since i am a planning Engineer in UAE. My Role needs to be office based most of the time with less physical work. Sometimes I felt overstressed sometimes on continuous work, but not regularly. I am also having regular practices of doing exercises, playing, jogging etc and I am more consciousness on my diet. I also detailed below the issues what i am currently facing for your better understanding.

During sleep time my breathing pattern gets disturbed(Regular event) i.e Inhaling but not exhaling(pause in the breathing) & vice versa, hence i got waked from the deep sleep. This occurs during my initial lying down at bed, turning my neck towards left or right side .After some times it may automatically subsidized and i am felt good in the morning. Also in between the deep sleep i faced this issue a couple of times(Last month).

During regular day time also i am facing these issue from last two days, but able to manage the same.

I am having tonsil , Dust allergy problem for which i have taken earlier medication(Not currently taking any medicine for the same). Can these be one the causes for my respiratory problem currently i am facing?. I am not having smoking & tobacco practices.I am not sure i am snoring ,since i am staying alone.

Based on the above illustration, can u Pls state me the next step on my diagnosis/treatment procedure, changes required in my diet, special exercises prior to sleep?

Thanks & Regards

Brief Answer:
We should definitely rule out obstructive sleep apnea in your case.

Detailed Answer:
Thanks for your follow up question on Healthcare Magic.
I can understand your concern.
So you are having more breathing difficulty in night time. An it is not seen with tonsils or allergy.
The way you described your symptoms, we should first rule out obstructive sleep apnea.
Most common cause for this is obesity.
So please let me know
1. Your weight
2. Your height
3. Do you have excessive fat in neck area?
Please reply me answers of above asked questions, so that I can guide you better. I will be happy to help you further. Wish you good health. Thanks.

I have gone through your photograph.
You are having double chin (excess fat in neck area).
You're weight I also more according to your height.
I can have calculated calculated your BMI (body mass index) and it comes around 32. And this brings you in grade 2 obesity.
So obstructive sleep apnea should be ruled out in your case.
So consult sleep specialist and get done sleep study.
Please let me know
1. Do you have excessive daytime sleepiness?
2. Do you have morning headache?

Brief Answer:
I still feel, your should get done sleep study.

Detailed Answer:
Thanks for your follow up question on Healthcare Magic.
I can understand your concern.
Well, I still feel, your should get done sleep study.
Or first consult ENT doctor and rule out other causes like tonsils and allergy (which are very less likely).
If ENT wise everything is normal, then sleep study is the only way to diagnose your problem.
Hope I have solved your query. I will be happy to help you further. Wish you good health. Thanks.
