Suggest Treatment For Dry Cough

Question: Dear Doctor,
My daughter's age is 4 years 6 month. She is suffering from dry cough from last 8 weeks now. Treatment is going on under a doctor (child specialist). There is some improvement but my daughter is still coughing severely on afternoon and night. She has completed several Antibiotic course during this period. She has no fever at all during this 8 weeks period. Other activities are so normal of my child. Her pulse rate, SPO2 is all normal.

According to Previous prescription on 11.03.2015 -

She completed Augmentin-DDS 4ml, 3 times daily for 5 days.
+ Grilinctus syrup 5 ml 3 times daily for 7 days
+ Montair LC Kid, 5 ml once daily at evening (continuing)

With this medicines, there was no such good improvement for my daughter. So doctor suggested me to do a blood test, chest xray and mantoux test for my baby. I will upload reports to you.

Report -

1. Blood test detected 12% eosinophils.
2. Chest xray detected - PERIHILAR CONGESTION NOTED.
3. Mantoux test - No infection detected. Absolutely normal.

After I have done all tests, my present doctor has suggested this prescription on 01.05.2015 -

+ Omnacortil-forte (15mg/5ml) - 6 ml 2 times for 7 days, then 6 ml once daily for 7 days, then 3 ml
once daily for next 15 days.
+ Forcan-syrup (50mg/5ml) - 7.5 ml daily after meal, for 5 days.
+ Lanzol-Jn - 1 tab once daily before breakfast.

But I am now afraid to continue treatment under him as I think he is unable to detect proper cause of the dry cough. I am afraid of giving my daughter such STEROIDS (Omnacortil-forte). I am in doubt now, if my present doctor is going into right direction or not. Please suggest me your thoughts on this matter and if possible please provide me some treatment solution.

I can understand your daughter 's situation and problem.
I have seen the attachments of blood report, chest x ray and doctor' s prescription.
She is having peripheral eosinophilia.
Following are the common causes for eosinophilia in children.
1. Allergy
2. Parasitic infection
3. Drug related.
Possibility of allergy is more as she is having dry coughing.
Parasitic infection of abdomen is common in children, but she is not having any abdominal symptoms. So this is less likely.
Drug related rise is also less likely as she was not taking any such drugs.
Get done serum total IgE level. It is more confirmatory marker of allergy.
And eosinophilic pneumonia does not present like this. So don't worry about eosinophilic pneumonia.
Better to consult pulmonologist and get done clinical examination of respiratory system and look for any abnormal sound like rhonchi or wheezing etc.
She may need inhaled corticosteroid instead of oral steroid.

Dear Doctor,

I have just received the serum total IgE report of my daughter. Please check the new attached document. The Ig E1 is higher than normal. Please suggest me what step I should take now. I also have discussed with my present doctor and both decided not to start the previous prescription medicines (discarding eosinophilic pneumonia) until we all are in a same ground. My daughter sometimes tell me that she has some abdominal pain. 1 month ago she has completed a course with medicine "NO WORM - 5 ml (total 2 dose) in a gap of 14 days".

It is to be noticed that she is coughing less than before from last 2 days. Though she is continuing syrup "Montair LC KID, 5ml everyday". Should I continue that? Looking ahead to your valuable suggestions.

I have gone through Serum IgE report.
It is high, so possibility of allergic cough is more in her case.
Please let me know
1. Have you find any association between coughing and allergen exposure? Allergen can be dust or pollens or animal fur or pollution.
2. Have you get done respiratory system examination of her for abnormal breath sounds like rhonchi or wheezing?
3. Is she having abdominal pain at present? Have you ever get done her stool examination?

Dear Doctor,

Following is the answers -

1. I noticed that she is coughing more if she gets some artificial smells, cooking smells of spices etc, dusts, very cold or very hot weather, air pollutions etc. In fact I already have these issues due to my "Histamine" related issues.

2. I never done any respiratory system examination of her so far. There is no abnormal sounds when she is breathing normally. But I noticed, when she is coughing, sometimes there are some short of sound involved.

3. She is only telling me (when eating food mainly) that she has little pain in her belly. Also I have never done a stool examination of her. It is needed mention that she had a dysentery 2 months ago with blood in stool. That was recovered within 2 weeks.

From last couple of days, she is coughing less than before. Also she is drinking water in a regular interval.

In my opinion she is having wheezing.
The sound you are talking about while coughing is mostly wheez.
So better to get done respiratory system examination to rule out wheezing.

1. Is she playful? I mean, is she having any sort of coughing or breathing difficulty while playing outdoor games?
2. Had she undergone ultrasound examination of abdomen at the time of dysentery?
3. Which kind of histamine issues you have?

Dear Doctor,

1. I misunderstood before about the respiratory system examination. Her present doctor has examined her few days back and then there was no sounds internally. But I understand that she is having wheezing while coughing more. She is playful but only indoor games due to lack of open field here. I noticed sometimes if she cry loudly or jumps frequently while playing, soon after that she coughs. But the cough comes under control quickly. Basically after taking some rest and drinking some water.

2. Her doctor did not done an ultrasound examination of abdomen at the time of dysentery. Doctor controlled dysentery with some antibiotics and digesting syrup. But it took 2 weeks and 2 complete course of antibiotics to get under control of the dysentery. IT IS IMPORTANT TO TELL YOU that she has started coughing after 2-3 days of the time of her dysentery started.

3. I have "Histamine Intolerance" probably a High Histamine Level. The extra histamine do not breaks with diamine oxidase. It was discovered by me during several years of seasonal treatment. I took inhaler (Slameterol & Fluticasone Propionate Inhalation IP 250) during winter. It is prescribed by my doctor. Also I took 1 "Levocet M" tablet daily. From this month I should stop this. And limiting my food to control it properly.

You might be having asthma.

Thanks for your follow up question on HCM.
By her symptoms, in my opinion, she will be positive for wheezing in respiratory system examination. So get done PFT (Pulmonary Function Test) to rule out obstructive pathology of airways.
As allergic coughing can be the only symptom of asthma in initial stages.
And about you I want to know in detail.
1. What are your symptoms of histamine intolerance?
2. Do you have breathing trouble? Because salmeterol and fluticasone inhaler is commonly used to treat asthma or allergic bronchitis.
3. Since how many years you are symptomatic? Since how many years you are taking levocet M?
In my opinion, you might also having undiagnosed asthma. Asthma is having familial tendency. So possibility of asthma transmission in your daughter from you is high.
So please reply me answers of above asked questions, so that I can guide you better.
