Suggest Treatment For Double Kidney Atrophy And Renal Cyst

Question: Dear doctor,

I have a friend, a 35 years old married lady, in China with Chronic kidney disease (CKD).She is on the 3 stage (GFR level is 41.76). She received treatment in China for about 5 years. However, the illness is still getting worse. The doctor in China told her that there is no pretty good method to make her fully recovered. It is even hard to stop the worsening.

I want to ask that is there any methods in the U.S. to treat the disease better. What’s the most advanced technique in XXXXXXX to treat CKD. She told me if she can get better treatment in the U.S., she will come here to get treatment.

Moreover, she learned that there are some new researches about treatment by using stem cells to cultivate kidney. Is this possible to be used for treatment now in America?

Finally, I want to know what medicine is best for her situation. Now her situation is Double Kidney atrophy, left renal cyst and double renal blood flow reduction. Can you recommend me some usually used medicine in XXXXXXX

Thanks for the query

It looks like your friend has advanced irreversible renal failure and this is how you should go about it

1. Time tested and effective treatment for retarding the progression of renal disease is by being on ACE/ARB group of drugs. I usually put my patients on Tab Losartan and increase the dose till the maximum tolerated dose. This retards the progression of kidney failure
2. Omega 3 fatty acids have proven to be beneficial. I prescribe three tablets daily
3. Salt and protein restriction prevents further damage to the kidneys. I ask the patients to restrict salt to less than 3g/day and protein to less than 1g/kg/day. A simple way to is avoid salty food and red meat
4. Stem cell transplant is coming up, I have a had a few patients who had undergone stem cell transplant. It did not reverse the disease but retarded the progression.
5. Make sure to keep the blood pressure under tight control of less than 140/90 mmhg

Renal transplantation is a good option however it is done in cases of patients who are in CKD stage V, it is not advised at such early stage.
