Suggest Treatment For Difficulty In Conceiving

Question: Hello,
I am 28 aged married women and trying to conceive for last few months. After trying some months I went to doctor. He did all test including HSG, blood, USG and my husband sperm count. All reports are ok except my TSh. My TSH was 8.3. So doctor prescribed me Thyronorm 50 per day. He told me to try 6 months and has given Gynogen Hp 75IU on D3, Clofert 50 from D3-D7 and Duphaston 10 from D16-D25. My period is normal and 30-31 days’ time. Doctor told me if I am not preganant he will start IUI. After 6 weeks of taking medicine my TSH is now 3.49 and 2 months failure. I am in big tension. Please help how to get pregnant.

Thanks for the query to hcm,
As all your and your husbands reports are normal , also the protocol being advised by your gynaecologist is very much acceptable for the cycles.
The present TSH of 3.4 is also within normal range now with 50mcg of thyronorm, so it is just a matter of time and you need to be patient with regards to the outcome. Stress can unnecessarily lead to hormonal imbalances and undesired failures.
As your levels have reached normal now so continue thyronorm and the protocol as advised and most importantly try to have intercourse during your fertile period which should be from day 12-day 20 of your cycle.
The increased frequency of natural intercourse during this period shall improve your chances of natural conception. you have already been given a time for total 6 months by your doctor which is to allow you to try naturally and if you are unable to conceive then IUI can be offered after ruling out cervical causes.
So please be patient and be optimistic. You are on the right track.

Dear Doctor,
Thanks for your reply. Should I continue Thyronorm 50 or reduce in to Thyronorm 25? When should I test next TSH? Should I test for T3/T4 also? How much cost of IUI and IVF in India? Is there any bad effect of Gynogen, Clofert and Duphaston in future? Kindly reply my question.

Hello,
You should get TSH LEVELS done every 3 monthly and you should not reduce the dosage of thyronorm as the present levels are because of the 50mcg dosage. Hence it has to be continued throughout even when you get pregnant and thereafter throughout the pregnancy.
IUI cost is fairly cheaper 5000-8000 rupees and IVF which is a different technique all together costs per sitting around 80000-1 lakh rupees.
The small dose of clofert , gynogen and duphastan will not cause any harm to your present hormone levels but would rather assist in timely ovulation. you just need to be stress free and don't think much about it and practice intercourse during your fertile period.

Dear Doctor,

Thanks for your reply. It help me a lot. I have just few more confusions. I am taking first day of menstrual cycle as D1. Am I right? I have exactly 30 days ovulation period so as per ovulation calculator D16 is my ovulation day. Right? Is there any general time for ovulation in a day? I mean generally when ovulation take place, morning, evening or night? We tried to do intercourse 2-3 times in a day after D14. Kindly reply me please.

You are absolutely right .
DAY 1 is the start of menses.
Secondly if your cycle is 30 days then as per calendar day 15-16 becomes your ovulation day but this is not fixed , hence a range of day 14-18 if often told to the patient. This can be monitored by LH ovulation kits from day 10 daily. The day the LH kit shows a positive result, within next 36 hrs the ovulation is likely. So thats the best way to monitor ovulation apart from follicular monitoring.
Thirdly, 2-3 times a day intercourse from day 14 is a good practice for couple who desire to conceive.

Is it possible not to see any symptoms before missed period?

Pregnancy symptoms can be experienced by many but not all before missed periods and they may range from nausea, headache, mild lower abdominal ache, even vomiting.
And yes sometimes pregnancy symptoms may even be seen late after even 6-7 week of pregnancy.
