Suggest Treatment For Diabetes

Question: I am diabetic -II for over 20 years, and now 65 year old. Unable keep up my sugar level. Earlier doctor put me on insulin 12unities per day and Amaryl M2mg per day . continued over six months .Then My sugar level frequently started dropping below 70 mg/dl .When consulted Diabetes care center doctor advised to stop insulin and put me on Glycinorm -M80 twice a day . But I am finding sugar levels are not normal. fasting / 120~130 mg/dl Post lunch / 165~200mg/dl.
I am confused which is better insulin or oral medication. If insulin taken for prolong period will it cause any harm. ?? Regards

Hi,
You seem to be having what is called brittle diabetes. Too low or too high sugars is a common problem in diabetics.
You need sustained and acting drugs whether oral drugs or insulin, so that sharp rises and falls are avoided.
Long term insulin is safer than any other diabetic drug; few side effects like weight gain and insulin lipoatropy may be seen but are not serious.

Depends on the severity of diabetes.
We have sustained release metformin, long acting insulins like glargine or detemer.
They act smoothly and slowly for 24 hrs duration and hence are sufficient as once a day dosing with little highs and lows.
