Posted on Mon, 20 Jul 2015
Question: I am 27 old male. 2 years back i got depresses due to work tension. I was diagnosed with ocd and treated for that. But instead of getting well i lost 8 kg weight in 15 days. Met another doctor he told i had anxiety and depression. I was treated with fluxitiene, modafiline and zepam. Got better but my irritation increased. I left the treatment after 4 months. After few months i got into depression due to work pressure. This time i was treated for few traits of borderline personality. Though i never had excess impulses or suicidal behaviour. I am treated with lithoril and qutipine since 1.2 years. I am good. But i got married and my wife fELT cheated as doctor told her that i might not be cured at all ans my treatment can last for whole life. Due to my medication which is used for bipolar disorder her family thought my doctor is hiding that i have bipolar. Disturbed with this she left me. Do i really have borderline? I am a professor in technical Institute and regarded one of the best teachers there. I have no record of misbehavior or any other thing. Please help me what do i have exactly. Are there any standard tests in XXXXXXX to see what exactly i have. I have been through my sisters death im childhood. Since then i am very emotional and sensitive person.

Brief Answer:
It seems depression primarily

Detailed Answer:
Hello,
Thanks for using Healthcaremagic.

Although I have not complete details of your symptoms and past history but considering your performance, achievement till date and good response with prescribed medication and your succinct way of writing there is almost nil possibility of borderline personality disorder.
Persons with borderline generally do not achieve this level and they get bothersome symptoms by adolescence.

The first possibility for me is depression but possibility of bipolar depression can not be ruled out.

Unfortunately there are no standard tests that can clearly point toward one or another diagnosis but there are tests which can be used for supporting clinical diagnosis. This applicable to XXXXXXX as well as any country.

For better determination it is better if you can visit psychiatry institute such as NIMHANS, Bangallore, IBHAS, New XXXXXXX CIP XXXXXXX Tezpur, Psychiatry Center, XXXXXXX etc.
They have the team of psychiatrist and psychologist and able to differentiate between minor things.
I am sure at these places you can get your exact diagnosis.

I hope this helps you.
If you have any further questions feel free to write back to me.
Thanks and regards

Brief Answer:
It is not curable but treatable

Detailed Answer:
Hello,
Thanks for using Healthcaremagic.

Bipolar depression is not curable but treatable which means you can remain normal with medication.

If medication stopped there is possibility of relapse in few months to years. Very rarely patient remain asymptomatic for years without medication but many others need continuous treatment.

Regarding marriage and relationship majority of them do well in supportive relationship but strain in relationship poses extra risk for relapse for them.

I hope this answers you.

Brief Answer:
Both are not same

Detailed Answer:
Hello,
Thanks for reverting back to me.

I understand your concern about lack of empathy of lawyers and impact on relationship.

Both bipolar disorder and borderline personality diisorder share some symptoms but they are not the same entity. One is personality disorder while other is mood disorder. The both have almost similar treatment and need almost life long treatment.
Many patients with Borderline show improvement after 40 years of age which diminishes need for medications.

Psychotherapy is option for borderline personality disorder. There is no doubt that Dr XXXXXXX is eminent psychiatrist and you can seek consultation from him. The other nearby option is NIMHANS XXXXXXX where comprehensive treatment as well as psychotherapy facilities are available.

i hope this assist you.
Thanks and regards

these are the things my wife said in her petition. frankly speaking i am on the treatment. but never faced such things. please read them and help me. do i really have to take such a high dose of lithoril 400 twice daily and quitipin. what are the possible side effects can i reduce the dosage. my doctor is reluctant in reducing the dose. i dont know why. i am trying my heart out. i go to gym. i relax by watching light hearted movies. most importantly i am sad due to the situation. but i am not giving up. being the only child i am taking care of my family. looking at legal aspect and teaching in my Institute. i feel time will help me. in past years i used to get such troubles but i used to get out of depression without treatment. now this whole stuff has created a havoc.

will it be possible so that i can talk to you over phone. just to discuss the issue.

Brief Answer:
Concealment is only ground not the illness

Detailed Answer:
Dear XXXXXXX
I understand your agony and stressful things.

As said by your wife lithium is used in bipolar disorder but is does not mean that is only indication.
There are around 15 conditions where it is used and that can be used as a defense.

I am sorry for concealing part as this is only ground for her as court repeatedly stated that mental illness is not a ground for divorce against what stated in Hindu Marriage act.
Since 50's there are numerous court orders which redefine the Hindu marriage act and you need to educate your lawyer about same.

Since currently I do not work in XXXXXXX and unable to take calls it will not be possible to talk to you over hone.

It will be better if you can talk somebody else who is practicing in your city.

Thanks and regards
Dr Ashok Kumar
