Suggest Treatment For Constipation, Difficulty Urinating And Groin Pain

default
Posted on Sat, 16 Jul 2016
Question: My father is already on Flomax for prostate issues. Last week having excessive difficulty urinating and terrible constipation with lower abdomen and groin pain that would shoot down into his penis. Went to Dr.'s office and Dr. said to double up on dosage of Flomax, take Miralax at night, Levoflaxacin for any inflammation, add more fiber to diet and gave a urinary test checking for infection and any blood in urine...which came back supposedly normal. Two night later we're in Emergency because no bowel movement for 3-4 days and was unable to pee that day. The were perplexed by the urine content test (gel and machine moved around belly) because wouldn't read as much as they thought he would have in him. Read about 13 Grams and they thought should be 1300. Wanting to relieve him, they gave him a catheter and it went through his prostate area completely smoothly to the XXXXXXX of the Dr.. Tests revealed pile of "poop" stuffed up in him, so they gave him an enema and sent him home with a huge jug of something to drink and clean him out. Since, just small amount of bowel movement, nothing indicating normalcy. Tonight, pain in lower left groin and throwing up. Has been able to urinate somewhat better for a few days. Says he feels like he has a gas bubble and is throwing up tonight and gurgling in the bowels. He thinks it's Diverticula (previously diagnosed) and nothing to do with his prostate. Just not sure and he doesn't want to go back to the emergency room. After throwing up a few times this evening, he took two Vicodin for pain. Since then, he's relaxed feeling less nauseated right now but worried about any lerking pain. He's on blood pressure medication and aspirin and has taken himself off of cholesterol medication last week and stopped taking the Flomax after returning from the hospital. Any suggestions or direction to go would be very helpful. He loves to self diagnose himself, of course.

Detailed Answer:
Brief Ans- UTI with Constipation with vomits lately/ Under Evaluation

Hi,Dear!! Thanks for the detailed query to HCM.
Read and reviewed the query in context to your health issues, which are
-Old Diverticulosis
-Bowel obstruction along with Flomax-which is lately stopped after catheterization
-Pain in lower left groin with vomiting
-has medicines for high BP and aspirin
with awaited lab/x-ray reports from hospital
In the awaited More details would be needed about bulging veins, for exploring further suggestions in your case.

My Impression-
In the given complex scenario,You seem to suffer from-Constipation with IBS which is from Colitis with accompanied diverticulosis.
The outflow urine obstruction you had,got relieved after easy catheterisation, proving Controlled BEP(Benign Enlarged Prostate).
Urine analysis and x-ray abdomen have come normal.

Comments on your complex progress of complaints-
Its only after giving enema, which relieved urinary retention and also non-responding Constipation. This constipation is the result of -Spastic Colitis+Alpha blockers for high Blood pressure and +Flomax(Tamsulosin) which lead to Constipation.
So your major problem was and is constipation-which complicated further after enema,to give lower left groin pain with throw ups- from severe aggravated infection-of diverticulosis you had. This pain and throw ups were from Aggravated Spastic Colitis with bacteraemia accompanied with it.
The pain which was causing reflux nausea/ vomiting, got relieved after-Vicodin, but the lurking low abdominal pain persisted.
And this could be from accompanied unattended unresolved Colitis you have.
X-ray abdomen is normal and possibility of ureteric stone is ruled out.

Reply to your specific question-
Q- Any suggestions or direction to go would be very helpful. He loves to self diagnose himself, of course.?

Ans-Dear, at advanced age of your father and with so much of complex un-diagnosed-situation,you need to target Colonic inflammation-which is lurking there from untreated and neglected old diverticulosis.

Do' Suggested-in given situation-
In view of the accompanying diverticulosis- I would suggest you to check with Surgeon and to rule out IBS with C from Spastic colitis, which needs immediate treatment with antibiotics(continue ofloxacin with Oz-orniadazole),
Stop Taking Flomax -to avoid drug induced constipation
Reduced anti-hyperensive from alpha blockers with suggestions from your Specialist Physician,
Be on Bland liquid veg diet,
Avoid spicy,fried, non-veg diet
Be on -Yoghurt /Cap Sporalc-probiotics- x 2 times a day,to stabilize colonic commensal micrbials.
Continue with antibiotics anti protozoala x 2 weeks time
Add PPI -Omprazole with Domperidone-to prevent nausea/ and vomits
Add Vit B12 and Folic acid
Gradually add low volumes of solids over next 10-15 days time.

Key ALERTS-
Consult Urologist to stop Flomax / Consult Physician to change blood pressure alpha blockers / Consult GI Surgeon and treat Colitis with accompanied urinary reflex urinary obstruction and retention.

Plenty of fluids and care of the Spastic Colon causing IBS with C is the main basis of correcting complex health issues your father has currently.

Hope this reply would reduce worries with your health issues and would help you to plan further proper care, with help of doctors there.
