Question: dear drs,

iam doagnosed with chronic fatigue syndrom 2 years ago

drs advised me to go to abtidpreasants which control brain chemicals
pls advise which one to take and its safer
motival or prozak?

I read your question carefully and I understand your concern.

It is true that sometimes antidepressants are used in chronic fatigue syndrome, although keep in mind that they are not the mainstay of therapy, cognitive behaviour psychotherapy and graded exercise are also important.

Regarding the two drugs that you need advice choosing between I would prefer Prozac. Motival is not one drug, it contains two components inside, Nortriptyline an antidepressant of the tricyclics class and Fluphenazine an antipsychotic. Prozac contains fluoxetine a SSRI class antidepressant. ssri in general have fewer side effects than tricyclics. In this case a combination of a tricyclic and an antipsychotic is even more likely to cause side effects. For that reason I believe Prozac to be safer and would prefer it to Motival.

tks for yr reply
i wanted to excercise and i want to move and make sport and be as i were before
but after one session of slow walking i will fatigued for 2 weeks and iam using the balance of the little power i have to do my work (commercial manager)
does antidepreasant help me workout if not i will not take
as i dont prefer to take these medicines. (what is yr opinion)

and if not what can help me to do the graduated excercise

I completely understand that exercise leads to subsequent fatigue, it is common in that condition. That is why it is important to have graded exercise, not extreme one. It can start with only 5 minutes a day, and if it is to be incremented has to be done very very slowly. It should be interrupted before it leads to tiredness, that is the key. Of course it is not that easy, actually it is thought not possible or effective in a percentage of patients but it has to be tried.

As for the drugs, no they will not help with the exercising I am sorry, there is no drug specifically for that. I believe your doctor has given them to you more for their anti-anxiety effect, as anxiety is a common accompanying issue. The whiskey detail you mention supports that, alcohol doesn't help with fatigue but does relieve anxiety which often is why people become dependent on it. So that beneficial effect of whiskey is probable due to it alleviating anxiety.

and what yr recomendation supplements to take to try to excercise

I must say I am not sure what do you mean by fatigue in front of your ears, it's not an area which exercises any activity and I do not see how it can be fatigued. I assume you mean pain or sensation of heaviness in the temples rather than fatigue. The explanation is somewhat difficult. You must understand that chronic fatigue syndrome is still an ill defined syndrome which causes and mechanisms have not been well elucidated. Even the name of the condition has undergone changes over the years. So I can not provide a clear explanation unfortunately, but it is among the diagnostic criteria of chronic fatigue syndrome that there is muscle pain, multiple joint pain, headache etc so those symptoms do not XXXXXXX me.
As for what vitamins, you shouldn't expect miracles from them, if you eat properly you are probably taking the necessary vitamin dose. There are anyway many over the counter vitamin supplements for different ages in the market, the most reputable brand is Centrum multivitamins, but other brands will do as long as they contain vitamin b1,6 and 12, calcium, vitamin d and c.
