Question: I lost a good amount of blood from hemmoroids, after a night of drinking and some drug use, not feeling dizzy or anything, still worried though

Hi XXXX! Good evening. I am Dr Shareef, a general surgeon answering your query.

As I presume, this incidence of blood loss from your hemorrhoids might not be the first time you had it, but possibly you might have had it earlier also. The effects of a major blood loss appears after a little while depending on the speed and amount of the loss. So not feeling dizzy or anything else might not necessarily imply that you were not affected by the loss. If I were your doctor, I would advise you with the following:
1) Get your pulse rate and blood pressure recording by your family physician, and report to an ER of your nearest hospital if advised by him.
2) As you consume alcoholic beverages and drug, the source of the bleeding not necessarily be solely due to the hemorrhoids. This could be due to the secondary effects of the primary insult to the liver by the alcoholic drinks. I would advise you to minimise or avoid any alcoholic beverages to prevent this kind of situation. Also drugs could bring the Hepatitis B virus and all other kind of dangerous infection into one's blood. So, it would be better to stay away from these. I am sure you are quite a grown up person to understand the implications if you do not abide with the advice.
3) To know the present status of your liver function, I would advise you for few blood tests like a CBC, ESR, LFT (Liver function tests), serum amylase and lipase, HbsAg test, blood urea, serum creatinine and a HIV test as a routine.
4) An ultrasound of whole abdomen to see the size and density of the liver and spleen if enlarged.
5) Refer you to a gastro enterologist if need be for a possible endoscopic evaluation of your GI tract to know the source of bleeding if not solely due to hemorrhoids.
6) If it was solely due to hemorrhoids, get the definitive treatment done by a general surgeon or a gastro-intestinal surgeon.
7) You would also need some modification in your diet towards more of roughage and enough liquids to avoid constipation worsening the existing hemorrhoids.
