It would be better if you could provide me clinical pictures as there is feature to upload photos on the right side of query page.

It could be in growing of toe nail or osteomyelitis of phalanx or chronic paronychia, but i would be able to say exactly only after the visual inspection at the lesion.

here I would advice you to see surgeon or dermatologist who would examine it to have a clear diagnosis.

Get back to me with images of the lesion.
