Suggest Treatment For Atopic Dermatitis

Question: Hi my daughter is 2.9 years old and suffers from atopic dermatitis condition. Tried all kind of medication with not much success. Stumbled upon Curel itch defense cream. Have been applying it since two weeks. Also, giving her Citrizine syrup. However, of late, her skin is breaking out and there seems to some kind of water coming out. Its not pus and does not smell. Also, there is no pain. There is extreme itching and it prevents her from sleeping. Kindly help. Is the Curel reacting with Citrizine? Why is her skin on her face breaking out and why is water discharge?

I am Dr. Kakkar. I have gone through your query and I have understood it.

She seems to have developed a flare-up of Atopic eczema.

Atopic dermatitis is an Immune mediated disease. Patients have an underlying dry skin. There is transepidermal water loss [TEWL] as a result of low water retentive capacity of the skin in atopic individuals and therefore skin becomes dry and eczema(rashes) prone, specially in dry, cold weather (in winters)

Atopic Dermatitis patients are prone to develop an eczema flare, in which the skin may develop dry, red, scaly, itchy areas/patches with or without oozing (Eczematous Dermatitis), which in turn may become secondarily infected as a result of scratching. Oozing suggests that there is secondary bacterial infection.

In children, atopic dermatitis may involve the face as well as the trunk, extensor arms and legs.

The cream that you have been using for her is just a moisturizer, which won't be effective for the flare-up of eczema.

Right now, what she requires is: an Oral Antibiotic (syp/suspension) to take care of secondary bacterial infection along with a Topical Steroid+Antibiotic combination cream, for the patches of eczema/dermatitis. These are prescription drugs and therefore, I would suggest that you seek an appointment with a dermatologist or a pediatrician, as soon as possible, and get a prescription for these. Your treating doctor might also put her on Oral Steroids for a very short period, depending on the extent of the rash(if it is extensive).

Atopic dermatitis is a steroid responsive dermatoses and therefore once she is treated adequately for her condition, she would improve dramatically.

Cetrizine may be continued because atopic dermatitis is an intensely itchy condition.

However, since the dermatitis may flare again, therefore, once the eczema settles down, you must continue with the moisturizer to prevent a flare.
