Suggest Treatment For Ascending Aorta Aneurysm

Question: Hi, I'm a 49 yr old male who was diagnosed with BAV in 2000 and recently in XXXXXXX of this year with a 5.0 cm ascending aorta aneurysm. I'm 200 lbs and in good physical condition and also a construction worker. I have moderate regurgitation but otherwise a strong and normal sized heart with no arterial calcification. My doctor is recommending a mechanical replacement of the valve as well as repair for the aneurysm. I am terrified of the Coumadin with my highly active lifestyle as well living in an extremely rural area so far away from a trauma center. I'm leaning towards pursuing a surgeon that specializes in repair of BAV's rather than doing the easier replacement surgery. At the very least, willing to put in the tissue valve instead of the mv. So far, what I seem to hear mostly is I'm too young for a tissue valve. I can't see how this mv and warfarin is going to work for me and I'm not willing to make the drastic lifestyle adjustment to accommodate it. As I said I'm a construction worker, lift weights, spend a lot of time outdoors , eat healthy and an avid woodworker. Not much there that is compatible with a life on warfarin. Your professional opinion and thoughts would be greatly appreciated. Thanks, XXXX

I take no medications for this condition or any other. Take only supplements such as multivitamins, fish oil and protein powders. Sorry about this info not making it on the patient profile.

Hi XXXXXXX Your concerns are understansable. If you so choose to have valve repair done instead of replacement you can go ahead. But, repair along with Ascending aneurysm repair is a major surgery and you might require to change lifestyle and stay near a Higher medical center anyway. The prosthetic tissue you are worried about will as it is be put in the Aneurysm repair. But you are perfectly justified in getting it done by a Surgeon specializing in the same. It is a big surgery and you should get it done at a high volume center where such cases are done regularly. So go ahead and get opinion of an experienced Cardiothoracic Surgeon for the same.
