Question: Hello doctor ,

Of late I have noticed that there are some changes going on in my body internally. I don’t know how to say , but whenever I hear unpleasant things I become very nervous and I begin to loose confidence and I am unable to face the situation . This situation occurs often nowadays frequently.
Is it anxiety or some hormonal changes .
My regular day begins @ 5 a.m and ends @ 11 p,m. also once I am back from Office I have to sit with my kids( aged 8 and 6 years) to teach them and this also involves lots of energy and shouting , and lots of mental action .
As a result there has been throbbing sensations at the back of my head also . Is it normal.
Any small argument with my sibling or my hubby makes me shiver inside , hands become very very cold for quite a long time and then it subsides automatically till another such situation arises .
Also there is one irritating colleague in my office who plays dirty politics which disturbs me a lot . Even though I am XXXXXXX to her I have to sometime compromise because she uses tactis to misguide my boss in my absence .
And then I start to think unnecessarily which I know I should not be doing as it will spoil my family life .
I don’t what to say to the doctor , but something is wrong .
Please advise what must be the problem and how to diagnose the problem .

It suggest GAD and social phobia.

Hello
I appreciate that you have nicely given brief history of your daily routine and stress.

Mostly it is GAD with social phobia.

GAD means generalized anxiety disorder.

The points are favor of it.

1) Nervousness.
2) Unable to face situations
3) Unable to face unusual thing easily and get nervous.
4) Loss of confidence.
5) Lack of proper concentration in work.
6) Somatic symptoms like headache and throbbing sensation.
7) Mental and physical fatigability.


All can arise because of your nature and temperamental development since born.

Some biological factor like neurotransmitter mainly serotonin and nor epinephrine can play major role in its development. Hormonal change does not make much difference.

It can be treated with medicines and psychotherapy.

1) Some natural ways to handle stress like Exercise, meditations, progressive muscle relaxation, deep breathing exercise etc will help.
Do stress free work and do pleasurable thing like music and hobbies.
Social and psychological support is must to increase your confidence.

Positive outlook and confidence will help to handle daily stress in work.


2) This condition can be treated with medicines like escitalopram/sertraline/peroxetine/clonazepam/etizolam etc.

But for that you have to consult psychiatrist and go for proper psychological evaluation

3) Psychotherapy like supportive psychotherapy and cognitive behavior therapy can be useful.

Principal of this is to face the situation and learn the response. After certain period of time you feel free in that situation but it will occur in hierarchical pattern.
