Suggest Treatment For Acute Back Pain

Question: Dear Sir/ Madam
I have a severe backache and it feels as if the nerve has been struck at my hip and the doctor says its muscular strain the reason behind may be of my son who is a CP child and is weighing 30 kg and we have to lift him for his movement and further the strain on my back and my hip is so severe that some times I am unable to make movement and i feel the strain of the pain from my hip to my toe.

Well, at present it does look like your muscle has gone into spasm and is compressing on the nerve causing radiation of the pain to the toe.

but it is surprsing the muscle relaxants have failed to work in your case. For now I would recommend you start with

1. Physiotherapy: Short wave diathermy+ Infra red will help

2. Use Warm water bottle over the affected area

3. Try some other tablet like Pregabalin( Lyrica) or Gabapentin. It will help soothe the nerve and the radiation of the pain.

Incase nothing works for another week, we may need an MRI of the spine

As recomended Medicine shall I have to stop the medicines which are being taken. and further today I have a swelling on my left knee on account of pain and nerve strain and even in the morning as I wake up its difficult for me to stand up on account of pain really its untolerable.

Oh the knee pain is a new finding. do you get the knee pain often. I would just like to know a few more queries to make sure it is not some sort of arthritis we are dealing with..

1. do you get frequent episodes of pain and swelling in joints like knees ankles?

2. when you get backache is it difficult to cha GE position at night?

3. any early morning stiffness at the back area?

4. any one in family with similar back problems

5. has any blood test been performed yet

For knee pain you can revising celecoxib. the muscle relaxanats are not a good choice for it and avoid sitting on the floor or cross legged position

Thanks for your query and I was late in replying you the same is on account of my doctor’s visit for medication at XXXXXXX for the same reason.

The reply to your query is as under.

1. No I do-not have the frequent episodes of pain and swelling in joints like knees ankles.

2. Yes in back ache it was really difficult to change the position at night.

3. Yes there was early morning stiffness at the back area.

4. My mother does have the osteoarthritis that too in the first stage as she is in the age of 70 years

5. Yes recently many of the tests were performed I will arrange to attach the reports of the same.

Further I have visited an Orthopedic for the problem and he has suggested the blood tests and medication which has been started by me. Please let me know what has to be done for permanent relief of the pain.

Well, The sort of complaints you are telling does look like going towards inflammation...That is some form of arthritis going on in the spine. However considering backache, it is a very common problem and we need to get to the bottom of it..

Firstly I would like to see the blood reports. I would like you to check for me if they have done ESR, CRP and HLA B 27 test...

Secondly, If you can get a MRI done it would be of utmost help, especially the screening for sacroiliac joint.

If both these are clear, we are at least safe it is not an inflammatory back pain but mechanical...Which can be handled with yoga and physiotherapy

Do let me know the results
