Suggest Treatment For Acidity And Burning Sensation In Neck And Belly
Question: I'm average build.. 172 cm.. 66 kg, no preexisted disease, addiction smoking, liquor consumption seldom, since long i've some trouble in stomach, irritating, gas filled belly, irregular latrine, acidity. medication taken some antacid like femacid 40 or pentoprazole.. now symptoms grows severe. belly frequently filled with gas, mucus with extracts, sometimes pain in right side of chest, acidity burning sensation to neck and in belly.. but no pain felt so far.. belly felt like heavy,.. bp normal, bs - normal, others are not tested..

Welcome to HCM . I am Dr Anshul Varshney. I understand your concern.
It seems you are suffering from Acid Peptic Disease. The cause of that or you can say triggers are alcohol and smoking.
I advise following in such situations:
1. To take a combination of Pantoprazole and Domperidone once a day instead of plain Famotidine or Pantoprazole.
2. To take a anti flatulent / anti acidity syrup which contains Simethicone. Very commonly used is Syrup Digene/ Gelusil.
3. To take soft and bland diet for a few days.
4. Walk after every meal.
5. Take frequent meals instead of 3 heavy meals.
6. Say NO to alcohol and smoking.
In case you have taken the combination of Pantoprazole and Domperidone earlier with no relief you can take a combination of Pantoprazole with Levosulpride available with name Pantocid-L once a day.
Follow these medications for 7 days. Even after that if your problem persists you would require Upper Gastro Intestinal Endoscopy.

I'm taking pantoprazole 40 with domperdome SR 30 combination and Gelusil mps 10ml 30 mns after each meal. It seems that the condition is improving but perhaps mucus is mixed with stool.. and gas is forming after each heavy meal.. but acidity is less, would you like to suggest any more medication because I have complain of irregular bowel.. sometimes it remains ok for days but sometimes it forms too much gas in belly in upper GI.. please suggest some medication for the long run and advise what to do in future..

I have two possible diagnosis for you:
1. Irritable Bowel Syndrome.
2. Malabsorption Syndrome like Celiac Disease
Now I am talking about this because change in frequency of stool is not an association with Acid Peptic Disease.
I advise you to get a test done. That is IgA Anti TTG Antibody. If this comes out to be negative I will treat you on lines of Irritable bowel syndrome.
So just get this blood test done and Revert back with report.
Till then continue with the same medications that you have mentioned.

I've consulted the local pathology shops nearby, but since the area I worked is so remote, this IGA Anti TTG test can't be done here. The first possible diagnosis for IBS might be suitable for me because I've no burning sensation in stomach only gas and irregular bowel clearance. Since I've started medications u advised, it helped a lot to stop acidity, but after meal gas formation is still continued. It is to mention that I've some problem with digestion because sometimes stool passing is irregular. Even from last 32 hrs stools was not cleared nor even i felt to go for big..
Please suggest diet routine and medication for IBS, I'LL let you know my status and if something get worse, provision for UPPER GI endosocopy is also there..
